Responsible for making rounds on all to assist nursing staff in resolving issues/concerns. Ensure there is a sufficient number of staff members and that the needs of the residents are adequately met. Communicate with staff, physicians, and patients point of contact when there are changes of the patient's condition or if immediate medical care is necessary. Review and evaluate your shifts work force. Monitor your shift's personnel to assure that they are following established safety regulations.Unit Nurse Manager, RN - Long Term Care or Short Term Acute/RehabFull Time Days - Monday through FridayOversee nursing operations and supervisory duties for entire unit. Ensures complete and prompt reporting of incidents with follow-up as necessary to Administrator and Director of Nursing.Unit Clinical Coordinator, RN – Long Term Care or Short Term Acute/RehabFull Time Days - Monday through FridayIn collaboration with Unit Nurse Manager, the Clinical Coordinator performs initial assessments and re-assessments of conditions and responses to treatment as well as any other clinical findings. Develops and implements plans to meet patients' needs, including pain management, wound/skin care and infection prevention. Educates residents/patients and regarding treatment measures. Promotes continuity of care through developing an individualized interdisciplinary plan of care in collaboration with the resident/patient, family and health care team.MDS Coordinator, RN - Long Term Care or Short Term Acute/RehabFull Time Days - Monday through FridayThe MDS Coordinator RN optimizes and coordinates the RUGS III process as well as MDS for non-Medicare residents.
