This office is located in Martinsburg, WV and provides service to the counties of Berkeley and Jefferson.Job Details Overview: As a Registered Nurse with us, you'll play a vital role in planning, organizing, and delivering home care services. We're seeking experienced nurses with a passion for community health education and engagement. You'll utilize community resources to tailor services that meet the unique needs of individuals and families within their homes and communities.Responsibilities:Conduct comprehensive assessments of patients and families at regular intervals to determine home care needs.Provide thorough physical assessments and medical histories.Continuously reassess patient needs and adjust care plans accordingly.Collaborate with physicians to develop personalized care plans based on nursing diagnoses.Involve patients and their families in the planning process.Implement preventive and rehabilitative nursing procedures.Administer medications and treatments as prescribed.Offer counseling and health education to patients and families.Identify discharge planning needs and implement plans prior to discharge.Perform peripheral short intravenous line establishment and administer IV medications under physician orders and with demonstrated competency.Evaluates patients referred for home care to determine appropriateness for admission. Accepts or rejects patients based on Agency's admission criteria.Establishes a home care plan including assessment of the appropriateness of the requested services.Assures that Agency is ready to meet the patient's needs by collaborating with Agency personnel on a regular basis.Explains Agency's policies and services to the patient and responsible family members during initial home care assessment.Completes documentation accurately, and submits to agency office in a timely manner including OASIS documentation.Qualifications:Graduate of an approved school of nursing.Currently licensed, in good standing, to practice in West Virginia as a Registered Nurse.One year community health nursing experience or equivalent professional experience.Knowledge of nursing and medical practices, procedures, regulations and guidelines pertaining to home care, including OASIS.Complies with accepted professional standards and practice.Skilled in interpersonal relationships.Demonstrates strong verbal and written communication skills as well as critical thinking skills.Demonstrates professional behavior, attitude and flexibility.Able to function independently, with minimum supervision.Job type: Full-timePay: $70,000.00-$75,000.00 per yearTRAINING PROVIDEDNote: This job description is not intended to be all-inclusive.
