The mission of the Colorado Coalition for the Homeless is to work collaboratively toward the prevention of homelessness and the creation of lasting solutions for homeless and at-risk families, children, and individuals throughout Colorado. The Coalition advocates for and provides a continuum of housing and a variety of services to improve the health, well-being, and stability of those it serves. The Patient Navigator assists patients in overcoming barriers they may encounter in obtaining eye care. This position is responsible for providing case management and navigation services to ensure patients receive recommended services and keep eye appointments. Essential Job Functions
- Integrates Patient Navigation services in partnership with existing Stout Street Health Center staff.
- Assists in scheduling appointments for eye exams and specialty services. Ensures all required medical notes and diagnostic test results are available at time of appointment.
- Assists in eye clinic flow and operation, while maintaining the upmost client care by providing relevant and appropriate health information and emotional support.
- Assists in arranging transportation and helping clients arrive at scheduled appointments on time.
- Monitors and maintains database for patients with serious eye conditions, including compliance with prescribed treatments.
- Works closely with volunteer staff of eye clinic, including doctors to achieve client's eye care needs.
- Assists with scheduling volunteer staff on a monthly basis.
- Monitors and maintains inventory.
- Coaches and supports volunteers and staff.
- Maintains current and complete COVID-19 vaccination(s), as defined by the Centers for Disease Control and Prevention and local health authorities.
- Performs other duties as assigned.
Qualifications Summary
- High School diploma or equivalent required.
- Two years of experience in a health care setting, preferably eye care, preferred.
- Bilingual (English/Spanish) preferred.
Knowledge, Skills and Abilities - Knowledge of health care and human service systems, community resources and programs.
- Practical IT skills, including use of Word, Internet, e-mail, and database input.
- Ability to organize and prioritize own workload and work within set time frames.
- Ability to relate to individuals who are from diverse ethnic and cultural backgrounds and who are experiencing homelessness.
- Ability to maintain confidentiality.
- Ability to pass automobile insurance carrier's motor vehicle record investigation.
