The salary range listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.Status: Full Time RegularShift: DAY SHIFTDepartment: Transitional Care UnitPosition SummarySummary:The Transitional Care Unit (TCU) Clinical Nurse Coordinator provides clinical leadership of a hospital-based sub-acute/rehabilitation unit, consistent with the philosophy and goals of Mount Sinai South Nassau, patient care department, and clinical service. The Clinical Nurse Coordinator develops and sustains an environment of excellence in patient/family service, growth & development of staff, and application of evidence based practice. The scope of practice encompasses the nursing process which is cyclic, overlapping and interrelated through case finding, health teaching, health counseling and restorative life and well-being. He/she is accountable for operations and clinical processes 24/7; oversight includes identifying and addressing variations in patient outcomes; clinical practices as well as proactive risk anticipation and fiscal stewardship with accountability for the cost effective; and efficient use of human, environmental and material resources.QualificationsResponsibilities:Education Required• Must complete in-house orientation program• Graduate of an accredited program for Registered Professional Nursing (Baccalaureate Degree in nursing preferred) • Must complete annual mandatory in-service education (Hospital/Nursing)Experience And Skills• Excellent interpersonal skills• Speaks, reads and writes English • Two years current SNF/Sub-acute Rehab nursing experience. • Demonstrated supervisory skills including charge nurse and preceptor experience. • Public relations, team-building, problem-solving, decision making, conflict management, analytical and critical thinking skills. • Computer skills.License/Certifications• Current and valid New York State registration authorizing practice as a Registered Professional Nurse (RN).• Certifications required for assigned area: • BLS • Gerontology or Rehabilitation RN certification • CPI Working ConditionsOther information:Working Conditions• Personal Protective Equipment (PPE) worn• Frequent, prolonged standing/walking • Lifts, positions, and/or transfers patients/equipment
