About the Role: The Access Coordinator assumes a pivotal role in patient care by orchestrating seamless connections to our services. Operating as a central conduit, this position is dedicated to the coordination of patient care, encompassing templating, scheduling, and precise management of patient communications. Qualifications: High School Diploma or Equivalent with relevant experience in healthcare scheduling, patient coordination, or a related field. Higher education in healthcare administration, business administration, or a related field is a plus. Equivalent work experience in a similar role may substitute for formal education. Attention to Detail: Demonstrated ability to handle complex scheduling with high accuracy and thoroughness. Organizational Skills: Strong capability in managing multiple tasks and priorities, including creating and updating templates and schedules efficiently. Communication Skills: Excellent verbal and written communication skills to facilitate clear and effective interactions with patients, clinical staff, and other stakeholders, including those from diverse backgrounds. Technical Proficiency: Familiarity with healthcare information systems and scheduling software. Experience with advanced scheduling tools and database management is a plus. Proficiency in Microsoft Office, particularly Excel, for tracking and managing data. Problem-Solving Abilities: Ability to troubleshoot and resolve scheduling conflicts and issues promptly while maintaining operational efficiency. Adaptability: Flexibility to manage frequent changes in scheduling needs and respond to urgent requests effectively. Responsibilities: Manage and coordinate patient scheduling through diverse channels, ensuring accuracy and efficiency aligned with LCHC's access model, which includes behavioral health and medical services. Create, manage, and continuously update complex scheduling templates, block schedules, and appointment structures with meticulous attention to detail. Handle a high volume of requests and adapt to frequent changes, including creating and modifying templates for various scenarios such as new hires and urgent requests, while ensuring strict adherence to organizational standards and guidelines. Facilitate seamless communication with patients and clinical staff to optimize scheduling and care outcomes, leveraging various communication modalities available during all operating hours, including chat, phone calls, and virtual pathways. Schedule patient visits in accordance with established guidelines and standards tailored to diverse care needs and specialties. Execute population health outreach efforts in alignment with Quality Improvement (QI) and Population Health strategies to enhance patient well-being and elevate quality of care.
