Learn about 4C's 4-day, 32-hour work week by clicking the link below.4C Health is a growing non-profit Community Mental Health Center currently serving the communities of North Central Indiana since 1975. 4C Health is deeply committed to increasing access to a continuum of care while integrating physical and behavioral health well-being for our staff and consumers.Position Summary:This position operates under the supervision of a center Health Service Provider in Psychology (HSPP) or Medical Doctor (MD) for staffing purposes. This position provides assessments throughout the agency to provide rapid access to care. This position works to provide mental health, addictions, and behavioral health clinical assessment/diagnosis within scope of practice as defined by degree and license. This position is responsible for participating in treatment planning with the center psychiatrist and client, providing treatment recommendations, maintaining adequate and timely documentation, and coordinating client's next steps in care. This position operates under minimal supervision and must exercise tact, judgment, and confidentiality in contact with the public, clients, and families, as well as fellow associates of the Center. The work is very broad in scope and requires a high level of professionalism.Duties and Responsibilities:
  • Provide mental health and SA assessments via telehealth and onsite (dependent on agency need).
  • Provide clinical client contact hours based on billable counts of service and FTE status as determined by Center policy.
  • Make recommendations and provide care coordination for ongoing care.
  • Complete client assessments that include intake or reassessment, evaluating a client's ability to meet the admission criteria for that particular department, identify presenting problem(s), history, medical information, family involvement, psychosocial/psychosexual information, and other pertinent information to form an appropriate clinical impression including substance use/abuse.
  • Complete a treatment plan, which accurately reflects the client's needs and ability to meet discharge criteria with goals and objectives that are time limited and behaviorally stated.
  • Ensure treatment plans receive all appropriate and necessary approvals and are staffed in a timely manner.
  • Document timely, accurately and in accordance with Center policies utilizing current systems and practices.
  • Complete the case consult, transfer and discharge summary processes per Center policy and provide any referral source information according to the needs of the referral source.
  • Complies with the supervision policy and licensing expectations policy of the Center.
  • Provide progress reports to referral sources as needed for each individual client.
  • Ensure that the appropriate documentation and paperwork is in place before providing services.
  • Provide emergency services assessments/crisis intervention and referral as needed.
  • Provide crisis intervention in clinics that do not have emergency services on-call systems when needed.
  • Attend continuing education seminars pertaining to specific population served, and to maintain licensure.
  • Provide telehealth treatment when needed.
  • Understand Center policy as it pertains to client care.
  • Abide by all Center policies and procedures.
  • Maintain positive working relations and rapport with the communities served.

Additional Compensation and incentives
  • Eligible for $150/month ($1,800 annually) in student loan repayment,available at hire

  • Master's Degree in Behavioral Health or related field (MSW, MS, MA) required, Licensed preferred.
  • Must have associate license and be working toward qualifying licensure, per Center policy.
  • If the QBHP is providing services in the addiction department must have or are working towards either state licensure (LAC/LCAC) or other national certification in addictions.
Experience:
  • Experience and knowledge of appropriate therapeutic practices preferred if direct line staff.
