Job DetailsJob LocationCHI - ACHH - Hughson, CASalary Range$20.00 - $26.88 HourlyJob ShiftGraveyardDescription36 HOUR PER WEEK FT NIGHT HHAHOURS: 7:00 pm to 7:30 am. 12 HOUR SHIFTS includes weekends and some holidaysSUMMARY: As a member of the multidisciplinary team, the Hospice Aide (CHHA) works under the direction of the RN Administrator and the supervision of the registered nurse in the provision of personal care, in order to maximize the comfort and quality of life of patients and families. Responsible for providing supportive care in accordance with the plan of care and Community Hospice and ACHH policies and procedures.RESPONSIBILITIES:1. The Hospice Aide assists with home-like environmental services such as homemaking services to preserve a safe, sanitary patient personal space and to enhance quality of life in accordance with the Hospice Aide assignment and the hospice plan of care.Standard: Daily rounds and activities are completed according to the hospice aide assignment and the plan of care.2. The Hospice Aide assists the patient with personal care and supportive services in accordance with the CHHA assignment and the hospice plan of care.Standard: Personal care will be provided exactly as planned and assigned by the registered nurse.3. The Hospice Aide will communicate with other hospice team members as needed to report patient changes and needs and requested changes in the care assignment related to patient changes.Standard: Communications with other team members will be done at the time of patient contact if urgent, and no later than change of shift.4. Documentation will be completed and placed in the patient chart.Standard: Hospice Aide charting will be completed by the end of each shift.5. Communicate with patient and family throughout the shift to provide care and encourage involvement whenever possible.Standard: Meet patient/family needs, and patient goals.6. Attend regularly scheduled care conferences (IDT) to review cases, problem-solve and cover unique issues, share professional support and exchange feedback aimed toward enhancing professional growth.Standard: Attendance at weekly conference encouraged, unless assigned essential patient care by the RN or Administrator.7. Attend death events and provide appropriate support at time of death to patient/family, as requested.Standard: Attends death events as requested.8. Participation in Community Hospice orientation of students.Standard: Mentors students, as assigned, and assists in the teaching of hospice concepts.9. Participate in inservices, workshops, and clinical meetings to promote professional growth and understanding of hospice care.Standard: Attendance at 80% of all mandatory inservices and meetings, designated as Hospice Aide appropriate, provided by agency.10. Follows all Community Hospice and ACHH policies and procedures including, but not limited to, safety, infection control, privacy and confidentiality, and HIPAA.Standard: 100% compliance11. Maintain a professional appearance of self and working area; exhibit appropriate behavior with guests and clients, i.e., courteous, thoughtful and professional at all times.Standard: 100% complianceSUCCESS FACTORS:1. Experience in providing homemaking and personal care services in the home setting highly desirable.2. Possesses and maintains current CPR certification.3. Valid California Home Health Aide Certificate must be maintained at all times.WORK ENVIRONMENT: Positions in this class are situated in a home-like environment where there is little or no exposure to variations in the weather or other similar elements.WORK SCHEDULE: Required to work a regularly scheduled 36-hour workweek (12 hours per day), for full-time employees
