Overview
MHA in Ulster County, Inc. seeks a Full-Time Registered Nurse to work in our Assertive Community Treatment Team (ACT Program) providing nursing care to individuals receiving outpatient services. The nurse works with the Program Director, Psychiatrist, Nurse Practitioner and Mental Health Professionals in the ACT program providing intensive mental health treatment, in a multi-disciplinary mobile team model, to 68 adults in Ulster County with a diagnosis of serious mental illness and possible substance abuse problems. They assist in the evaluation process and treatment of individuals referred as well as provide a range of outreach, treatment, rehabilitation and support services to meet the unique needs of each individual receiving services.
The schedule for this position is Monday - Friday 9am - 5pm.
The pay rate for this position is a range of $37-$42 hourly based on experience.
Additional pay differential for rotating on call shifts usually once every six weeks.
Responsibilities Include
Assist in the evaluation process and treatment of individuals referred to the Assertive Community Treatment (ACT) Program.
Provide a range of outreach, treatment, rehabilitation and support services to meet the unique needs of each individual receiving services from the ACT Program. Services are provided in a mobile multi-disciplinary model.
Conduct mental health and physical health assessments on individuals served by the ACT Program. Make all necessary medical referrals.
Provide management and administration of medication in conjunction with the ACT Program Psychiatrist.
Ensure all paperwork, documentation, QA requirements and billing needs are met.
Participate in ACT Treatment Team meetings, Incident Review meetings, Utilization Review meetings, Treatment Plan Review meetings, and other meetings as needed.
Collaborate with and coordinate client care with other ACT Program Staff.
Provide liaison with other agencies involved in care of clients.
Qualifications and Skills
A license to practice as a Registered Professional Nurse in New York State.
Working knowledge of the principles of biological and social science as they relate to anatomy and physiology, medical regimens, theory and practice of preventive and mental health care.
Skills required to develop and implement a plan of mental health care to meet the needs of individual patients, ability to recognize changes in client's overall condition and in signs and symptoms that warrant the immediate attention of a physician.
A clean, valid NYS driver's license that will meet and maintain a satisfactory driving record consistent with agency and insurance standards.
