Job Description:

A social services organization in New York City is looking to fill an immediate need with the addition of an Team Leader to their ACT team. In this role, the ACT Team Leader will be responsible for supervising and providing multi-disciplinary clinical and case management services delivered by a team of clinical and administrative staff with focus of assertively engaging hard-to-reach consumers with serious mental illness.About the Opportunity:

  • Start Date: ASAP
  • Assignment length: ongoing (Contract to Hire)
  • Schedule: Monday to Friday
  • Hours: 9am to 5pm (1-hour paid lunch)

Responsibilities:The ACT Team Leader will:

  • Manage day-to-day clinical operations and required productivity standards
  • Direct and coordinate consumer admission process, treatment and rehabilitation services, support services, and discharge process
  • Establishe and implement procedures for monitoring of workflow and promoting enhanced staff performance
  • Assist with conducting other required assessments with clients, such as Child and Adult Integrated Reporting System (CAIRS) reports, Assisted Outpatient Treatment (AOT) reports, and Home and Community Based Services (HCBS) eligibility assessments
  • Provide direct supervision of team members, handles complaints and grievances
  • Monitor service delivery and impact of service effectiveness as well as consumer satisfaction
  • Schedule staff work hours to ensure appropriate coverage, monitors and approves timesheets
  • Assist with scheduling 24-hour crisis intervention on-call services on rotating basis
  • Participate in staff recruitment, interviewing, hiring, orientation and performance plans
  • Ensures the use of evidence based treatments in the areas of mental health and substance abuse to address client needs (e.g., cognitive behavioral therapy, harm reduction, motivational interviewing, trauma-informed care)
  • Perform other duties, as needed

Qualifications:

  • Master's Degree
  • LMHC, LMSW, LCSW, or equivalent in licensure in NYS
  • 2+ years of experience working in Human Services with individuals with serious mental illness and those with serious mental illness and co-occurring substance use disorders
  • Able to work in the Microsoft Office Suite, should be proficient in Word, Outlook and Excel
  • Must be outcome oriented in a fast paced environment with deadlines

Desired Skills:

  • Knowledge and experience of electronic health records (specifically AWARDS by Footholds)
