GENERAL STATEMENT OF RESPONSIBILITIES

Schedules and coordinates activities for individuals, both in their home and in the community. Additionally, plans specific activities to support individuals in reaching their personal growth goals. Communicates and coordinates all issues/information related to these activities. Reviews progress data and reports on outcomes toward goal areas. Supports culturally competent recovery-based practices and person-centered planning as a shared decision-making process.

QUALIFICATIONS

Education

• High School Diploma or GED preferred
• CMH/DMH Training preferred

Experience

• One year experience working with people with disabilities
• Character of high moral standards as stated in licensing regulation
• Three references deemed acceptable to Spectrum
