Position Summary: Lifestyle Coordinator plans, organizes,facilitates and directs recreational activity programs for residents in the AssistedLiving Community. Prepare for events by obtaining and organizing necessarymaterials for activities. Directs andorganizes such activities as sports, dramatics, games and arts and crafts toassist residents to develop interpersonal relationships, to socializeeffectively, and to develop confidence needed to participate in groupactivities. Regulates content of programin accordance with individualized patient capabilities, needs andinterests. Organizes and coordinatesspecial outings and may accompany residents on outings. Plan and document a monthly activitycalendar. Participate in assessing anddocumenting progress charts and periodic reports for medical staff and othermembers of the treatment team reflecting resident reaction and evidence ofprogress or regression. Network withcommunity organizations, businesses, schools and individuals to form a solidvolunteer program. Participate inplanning and implementing internal and external Community events.

Work Environment: Thisjob operates in a professional office or home office environment depending onlocation. This role routinely uses standard office equipment such as computers,phones, photocopiers, filing cabinets and fax machines. Operate a car orcompany vehicle for required travel.
Physical Demands: The physical demandsdescribed here are representative of those that must be met by an employee tosuccessfully perform the essential functions of this job.The physical demands of thisposition include;
- Standing, sitting, walking, changing ofposition, reaching, extending hands & arms, reach across midline,crouching, balancing, kneeling, stooping, bending, lifting 25 lbs. from sit tostand & push, pull, lower 100 lbs. and push resident in wheelchair 350 lbs
Do notattempt to lift residents or support entire weight of resident.Lowerresident to floor in the event of fall. Utilize two man lift procedureUtilizerequired safety equipmentSensory Demands: •Auditory: to hear and understand verbal request, •Visually: be able to read necessary information to completework tasks, •Verbally: communicate with customers and others on your teamSafety Equipment: •When transferring a resident, gait belt, lumbarsupport, two man lift policy and use of mechanical lifts in notedfacilities. •Use of PPE is required when appropriate toprevent exposure to bloodborne pathogens. •Use of seat belt when driving or riding in avehicle, no use of cell phone while driving. •Mandatory reporting of all work-related injurieswithin 12 hours to supervisor with participation in required documentation. •Participation in our drug testing procedures isa mandatory part of employment with this Community.
OSHA Exposure: Category 1,possibility of exposure on daily basis. Requireduse of personal protective equipment
PositionType/Expected Hours of Work: This could be a full-time or part time position.Hours can vary depending on coverage and demands of the community.
Travel: Some travel will be required to provide offsite activitiesfor residents and attend organizational meetings. This position will alsobe expected to assist with transportation of residents to and from appointments
Qualifications/ Education/Experience/Skills: Education :
- Minimalhigh school graduate and minimum age of 18 years of age
License/Certifications:- Current Driver's license required
