Reports directly to the Clinical Services Director.SpecificationsEducation: High School Graduate or GED minimumExperience: Minimum 1 years' experience in working with the elderly in a social or recreational program. Providing or assisting with coordinating services for a frail elderly population within the past 5 years preferred.Number and Type of Employees Supervised (optional): NoneLicensure, Registry or Certification: NC Certified Nursing Aide (CNA), National and/or NC Activity Director, or NC Licensed Recreational Therapy Assistant (LRTA) Certifications preferred, but not required. Special Training: Must be able to work effectively in a team environment. Good understanding of psychosocial problems of aging. - Collaborates with the Activities Coordinator, lead CNA's and Recreation Therapist (RT) as appropriate to help develop and implement specific programming as part of the plan of care for participants in the Adult Day Health Center to include recreational and social activities.
- Documents participation in the participant's medical record.
- Helps plan and implement both group and individual activities based on participant interest and in collaboration with the Activities Coordinator, RT and Center Lead CNAs.
- Communicates any changes in participant condition to IDT team members.
- Builds within the Center a sense of community and a warm social atmosphere in which participants are able to identify a support system.
- Helps plan a monthly/daily calendar for the Adult Day Health Center based on participant interest and in collaboration with the Activities Coordinator and Center Lead CNAs.
- May collaborate with marketing team and other disciplines to develop the Senior Total Life Care quarterly newsletter.
- Assists with monthly caregiver support group, Participant Advisory Council (PAC) and other specialized group activities as planned.
- Participates in Quality activities as appropriate.
- Coordinates with various community resources.
- If CNA certified or appropriately trained, may help with center/home personal care services which may include on-call during the weekdays/weekends and providing personalized care in the home.
- Supports Senior TLC's mission to encourage and support the quality of life of seniors wishing to continue living in the community; its vision to be the preferred provider of individualized care for seniors in the community; and its values of respect, integrity, accountability, compatible goals, and compassionate care.
