Plans, coordinates, and provides therapeutic and case management services for residential clients and their families.

JOB DUTIES AND RESPONSIBILITIES:

1. Meets with assigned clients within 24 business hours of admission to begin the process of developing a therapeutic relationship as well as the client's treatment plan, and discharge plan.

2. Provides individual counseling for at least one hour per week.

3. Engages the family/supports in the client's treatment, with consent of the client. Conducts family

sessions as clinically appropriate.

4. Provides group counseling—small group therapy, large groups and educational groups, as assigned.

5. Reviews SUD and tobacco medication assisted treatment (MAT) options with the client and documents recommendations with clinical rationale, the client's reaction and the plan for referral to the medical staff, if appropriate.

6. In conjunction with the client and aftercare specialist, develops a comprehensive discharge plan that includes therapeutic treatment for SUD at the appropriate level, therapeutic treatment for co-occurring psychiatric and/or medical conditions, and safe and appropriate housing.

7. Provides case management duties as needed for assigned clients.

8. Completes all paperwork, in a manner consistent with professional, regulatory and payer standards, on time, to ensure proper documentation is in chart. This includes:

• Treatment Plan completed within 6 days for both detox and residential admissions

• Treatment Plan updated 15 days following the comprehensive treatment plan

• Individual sessions occur at least weekly, and the documentation of the session is completed within 24 hours of the interaction

• Group notes completed by the end of the week in which the group occurred

• Family contact or family session notes completed within 24 hours of the interaction

• Medication Assisted Treatment note completed within 24 hours of the interaction

• The discharge summary must be completed within 5 calendar days of discharge

• Client information summary (CIS) completed within 5 days of discharge

• ASAM completed according to utilization review schedule as established by funding

9. Collaborates with other agencies, such as probation/parole and children and youth services, with the client's permission, including preparing reports and correspondence as indicated.

10. Assesses clients and works with families in a professional, thorough and timely fashion, assuring that the client/family has a clear understanding of what is recommended, including alternative options, and the rationale behind the recommendation.

11. Triages clients who require services other than outpatient (i.e. Mobile Engagement Services (MES), Crisis, Mental Health services) in a timely and organized fashion and makes referrals as indicated.

12. Participates in daily morning report and able to share pertinent information about each assigned client, including but not limited to, the client's insurance coverage, anticipated day of discharge, and anticipated discharge plan, including aftercare and housing arrangements.

13. Maintains confidentiality of all materials handled within the Network/ Entity as well as the proper release of information.

14. Complies with Network and departmental policies regarding issues of employee, patient and environmental safety and follows appropriate reporting requirements.

15. Demonstrates/models the Network's PCRAFT values during interactions with all customers.

16. Demonstrates Performance Improvement in the following areas as appropriate: Clinical Care/Outcomes, Customer/Service Improvement, Operational System/Process, and Safety.

17. Demonstrates financial responsibility and accountability through the effective and efficient use of resources in daily procedures, processes and practices

18. Complies with Network and departmental policies regarding attendance and dress code.

19. Demonstrates competency in the assessment, range of treatment, knowledge of growth and development and communication appropriate to the age of the patient treated.

20. Other duties as assigned

EDUCATION:

• Master's Degree in Counseling, Psychology or Social Work preferred. Minimum Bachelor's Degree in a human services field and/or certification as an Addictions Counselor by a national or statewide substance abuse counseling certification board and equivalent work-related experience.

• Meets Department of Drug & Alcohol Programs (DDAP) standards for a “Counselor.

TRAINING and EXPERIENCE:

• Clinical experience in the provision of AOD clinical services and/or mental health services.
