Come join our team!!** A dynamic RN Manager position is now available to oversee and manage liaisons at area hospitals and the intake department for the Eddy Visiting Nurse and Rehab Association! This role is responsible for screening, processing, and coordinating care from area providers for home care services. Includes oversight of a team of RN /LPN liaison, coordinating coverage and dates of assessments, and working closely with the CHHA teams. Ensures that information gathered for services is accurate, relevant to home care services and processed timely meeting regulatory requirements. Position requires extreme organization, collaboration, and able to articulate to providers what is required. This individual will need to strive to ensure a smooth transition of care is provided and all necessary information is received so that the admitting clinician can provide a safe admission. This position does not require direct patient treatment. Requires the following: + Knowledge of homecare regulations + Experience with referrals, as this position assists with referrals for Empire Home Infusion Services and Northeast Home Medical + Knowledge of IV coordination a plus. + Strong communication and computer skills a must. Work scheduled is full-time, Monday - Friday Pay Range: $44.55 - $58.45 Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates.
