**Admissions Concierge**
**Schenectady Center** is actively seeking an **Admissions Concierge** for our Skilled Nursing Facility in **Schenectady, NY**. The ideal candidate will have prior experience in a Nursing Home Admissions department, and a thorough knowledge of all local hospital systems.
**DUTIES:**
* Welcome new Residents & their families to ensure a smooth, hassle-free move
* Ensure rooms are clean & ready before welcoming new Residents
* Introduce new Residents & their families to life in our Home and show them around the Facility
* Coordinate between new residents & unit managers to ensure their quick evaluation upon arrival
* Follow up with new residents within three days of arrival
* Make at least three round per day on every floor to discuss Residents needs and concerns
* Maintain a daily log with questions, concerns and issues raised; and any follow-up steps taken
* Submits Daily logs to the Facility Administrator
* Assists Residents with the discharge process & follows up the next day
* Establishing & maintaining relationships with local hospitals, physicians & community organizations for the purpose of generating consistent admissions to the facility
* Understand the admissions process including policy, procedures, assessments & paperwork requirements
* Support the facility's PR to generate a positive image & encourage word-of-mouth referrals
* Maintain knowledge of regulations governing Medicare/Medicaid/Managed Care reimbursement processes
* Communicate special needs of new admissions to staff to ensure a smooth transition
* Provide appointments for facility presentations and on-site tours
**REQUIREMENTS:**
* Demonstrate passion & enthusiasm to work with elderly Residents
* Minimal high school diploma or equivalent
* Excellent interpersonal & communication skills
* Ability to multi-task as well as stay on task & concentrate with constant interruptions
* Prior experience as post-acute and long-term care Admissions
* Experience in community relations preferred
* Be self-driven with excellent internal sales/marketing skills
* Ability to implement the process to achieve desired outcomes
* Possess communication, time management, and organizational skills
* Solid computer skills; working knowledge of MS Office
* Nursing License preferred
