Qualifications include:
- Bachelor's degree in Human Services required
- Working knowledge of OPWDD programs, regulations and admissions processes for all programs offered at Life's WORC
- Knowledge of Microsoft Office Suite: Excel, WORD, Power Point is required
- Excellent written, communication and organizational skills
- Valid driver's license
Responsibilities will include:
- Collect and verify that all admissions documentation (i.e. application, Life Plan, DDP2, psychological, psychosocial, physical, etc.) is included in program referral packets
- Coordinate and conduct screenings and pre-placement program visits/trials as well as prepare the team and their families prior to admission
- Ensure all functions related to admissions, discharges and transfers for all programs offered at Life's WORC are completed according to Life's WORC's policies and procedures
- Demonstrate high quality customer service and communication throughout the admissions process
- Ensure regulatory compliance with all admissions, discharges and transfers for all programs
- Electronically log and track progress of all referrals
- Prepare Non-Residential Admissions & Discharge meeting agenda for bi-weekly meetings
- Record meeting minutes for Residential Admissions & Discharge Meetings and maintain accurate Admissions data (i.e. referrals, vacancies, etc.)
- Distribute acceptance/decline letters to families after dispositions are made
- Enroll all accepted individuals in their program in Medisked, including uploading all pertinent documents
- Ensure completed referral packets are scanned and uploaded into the Intake folder
- Coordinate discharge process with the family, program, and OPWDD as needed
- Function as a liaison with state and local government agencies (i.e. OPWDD, SSA, DSS) to ensure eligibility and benefits are active
- Perform outreach and coordination activities with OPWDD, families and other outside service providers to enhance outreach efforts and increase referral opportunities
- Travel to all program sites (as needed)
