Review all referrals submitted by hospitals for admission to rehab and/or LTC

Keep record of LTC applications, and follow up with family inquiries regarding placement, as well as billing to assist in payment options.

Open communication with hospital discharge planners on acceptance or denial of patient referrals

Ensure required referral information and data are obtained timely prior to an admission arriving

Entering patients in the EMR

Follow up daily on all pending referrals

Bridge communication with the team regarding new admissions

Basic understanding of insurance, electronic medical records and medical terminology preferred

General use of Microsoft Office Tools

This role will also learn basics of MDS to assist the Care Coordination Group as well as other duties to complete the team goals.Qualifications:Current nursing license (LPN/RN). Must have a genuine interest in geriatrics and assuring quality medical records on residents. Ability to adapt to frequent changes in routine. Minimum of one year in medical field and computer experience preferred.Physical Requirements: Must be able to lift up to 50 lbs. Ability to walk, climb stairs, push and/or pull equipment and residents as needed. Prolonged standing or walking. Able to maintain high energy level throughout an eight hour shift.
