This is an external business getting position, combined with Admissions responsibilities. You will be in the hospitals (and other locations/centers) every day to secure/facilitate new admissions. The remainder of the day you will be at the center, coordinating the admissions process for new incoming residents.A little more about our center:The Terrace is a classic Skilled Nursing Facility that has 115 beds, with a heavy emphasis on long term resident care and a small, but growing, short-term rehab unit. NOTE: we are one of the only nursing homes in the area with specialized dementia programming.We have a busy Activities Department that keeps our residents engaged with low level sensory rooms, cognitive therapeutic games, regular entertainment and cooking classes.Check out our 5-Star Google reviews!Job DescriptionOfficially we title this job an Admissions Coordinator, but in other centers it may be called an Admissions Rep or Intake Coordinator, Marketing Representative or Liaison.What you will do in this role:You will be responsible for daily census building activities of one of our Skilled Nursing Facilities (SNF), in Waukegan. This role is a combination of out-bound marketing activities and inside admissions duties. You will spend 30-50% of your time out of the center meeting with referral sources and doing bedside visits in the hospitals.Your number one goal is to achieve our budgeted census in this Skilled Nursing center.To do that, you will need to create & engage in out-bound business getting activities. These may be cold calls, hosted events (like CE presentations), Lunch-n-Learns at a variety of locations or even center-based events meant to increase tours to the center. You'll need to be outgoing & organized.You should be self directed in your efforts, and have the ability to manage relevant CRM metrics to ensure measurable results are attained. Example: Within the first 6 months we would expect you to increase our number of sources by 15% and increase our overall referrals by 25%.This position will not have any direct reports, but will be intertwined with administration, our central admissions partners & our regional marketing team.After you drive in the referrals, now you need to transition them into an admission.Picture this:When you are a patient, the transition from hospital to SNF is stressful. As an Admissions Coordinator, you will help to organize multidisciplinary services, so the patient/resident can safely transition into our center. One moment you may be analyzing the cost of meds, the next could be touring a family member, and the next is ensuring your intake packet is correctly signed.You will need to have strong organizational and time management skills to be successful in this role. (You will be expected to rapidly expand our census.)Bilingual (Spanish) is ideal, but not required. This center has targeted Spanish speaking programs. So, it will be helpful when interfacing with families.Experience with Point Click Care (PCC) and a CRM is idealWorking knowledge of federal & state regulations, along with reimbursement & insurance verification experience (Managed Care, Medicare & private insurances)Associate's or Bachelor's Degree in a relevant healthcare field. Our ideal candidate will have experience as a Discharge Planner, Social Services, nursing or Admissions within the SNF setting.
