Admissions LiaisonRegistered Nurse or Social WorkerNow hiring for Full Time on the 9:30am-5:30pm shiftParker Jewish Institute for Health Care and Rehabilitation, located in New Hyde Park, NY, is seeking a full time Registered Nurse or Social Worker to join our team in Admissions and serve as a subject matter expert, acting as the single point of decision making for patient admissions.This position is an exciting opportunity for a Registered Nurse or Social Worker to perform pre-admission clinical assessments of referred patients and to collaborate with our outstanding interdisciplinary team, providing outstanding clinical judgement to admit patients to our 5-star CMS rated facility, allowing Parker to focus on its outstanding patient centered care and allow us to continue to raise the bar in clinical excellence. The RN or Social Worker in this position will play a key role by completing pre-admission assessments (PRI) in conjunction with the Admissions Assistant Vice President, to identify medical, social and financial issues with potential new admissions, and ensure that patient's needs will be met upon admission by collaborating with the Parker team. You will act as an ongoing patient liaison, serving as the initial contact for patients coming into Parker, while maintaining ongoing metrics and quality improvement initiatives. Position Requirements
- Current and valid Registered Nurse license in NYS or NYS Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW)
- Experience in a hospital setting and/or skilled nursing required
- Experience with admissions, discharge planning and case management
- Knowledgeable in PRI and long-term care reimbursement issues
- Ability to work independently and with a team across a variety of disciplines
- Availability to work alternating weekends
- Fully vaccinated against COVID-19
