The Admit, Discharge, Transfer Nurse is to ensure a smooth operation of the unit while providing admission assistance, managing discharges, and facilitating patient transfers. Duties include but not limited to:
* Colaborate with nursing staff to prepare documentation and education for discharging.
* Help the patient transition from the hospital to home by providing verbal and written information about their medical care and next steps.
* Interact with physicians, nurses, other departments, patients, and their families to ensure all needs are met for discharge or transfer.
* Performs the duties/responsibilities of a staff medical surgical nurse, considering needs specific to the standard of care for patients, within the scope of practice.
Additional Job Description
Minimum Training and Education: Bachelor of Nursing degree preferred. A minimum of one year of work experience as a registered nurse required.
Required Licensure, Certifications, Registrations: Licensure as a registered nurse by the South Carolina Board of Nursing or a compact state. Current Basic Life Support (BLS) certification from an American Heart Association (AHA) is required.
