SUPERVISOR: Supervisor for Adult Care Managers HOURS: 8:00- 5:00, M-F flexibility as needed

QUALIFICATIONS:
* Bachelor's degree in health or social sciences and one year of related experience or, Associates degree in health or social sciences and two years of related experience,
* Reliable transportation
JOB SUMMARY: The Adult Care Manager facilitates a planning process that builds on the patient's strengths, supports and promotes stability, wellbeing and self-sufficiency. Ensures that individuals are informed decision-making participants by establishing and coordinating an interdisciplinary team that includes medical, behavioral health, and other community providers.
Essential Job Duties
COMPREHENSIVE CARE MANAGEMENT
* Maintain assigned caseload of forty - forty five adult individuals
* Complete comprehensive assessment identifying medical, behavioral health, chemical dependency and social needs.
* Develop a person-centered plan of care that integrates service needs and identifies goals.
* ? As goals are met and needs decrease, prepare the individual for graduation from the program.
HEALTH PROMOTION & CARE COORDINATION
* Support adherence to treatment recommendations.
* Link to needed services to support well-being and goals.
* Advocate on behalf of the patient to ensure all care team members are informed of current treatments and preferences.
* Escalate care management to practice-based resources when clinical assessment is needed.
* Coordinate with treating providers to assure that services are provided and that changes are addressed.
* ? Monitor, support and, if indicated, accompany the individual to scheduled appointments.
COMPREHENSIVE TRANSITIONAL CARE
* Follow-up with notification of admission and/or discharge to/from an ER, hospital, residential, or rehabilitative setting to assure follow-up appointments are scheduled, ordered medications have been received, and other needed services are in place and provided to avoid gaps in care.
* Link with Jericho Road nurse and/or clinical pharmacist for support while transitioning between care settings.
SUPPORTIVE SERVICES
* Utilize peer support, support groups and self-care programs to increase the individual's knowledge about their needs, engagement and self-management capabilities, and to improve adherence to prescribed treatment.
* Identify available community-based resources and link child and family with community supports as needed.

PHYSICAL REQUIREMENTS: Must be able to sit or stand for long periods of time. Exert up to 50lbs. pushing, pulling, or lifting. Walking, climbing stairs required. Able to talk/hear and communicate with others with or without reasonable accommodation.
TRAVEL REQUIREMENTS: Must be able to travel to sites while maintaining standards of safety and compliance with motor vehicle traffic laws. This position may require travel for necessary training and conferences.
