Services provided are primarily in the community and client's living environment. Services may include skill building, modeling, coaching, coordination and connection to resources and natural supports. Position works within a team to navigate and meet the comprehensive needs of the client.ESSENTIAL RESPONSIBILITIES
- Provide regular and consistent services and supports to clients identified appropriate for community supports.
- Management of all aspects of clinical treatment including referrals to additional resources, crisis assessment and support as needed
- Work with client on skill building, activities that support stabilization in living environment, connection to community resources and natural supports
- Service provision shall be conducted primarily in the client's living and other environments as necessary or appropriate
- Provide outreach and engagement when needed
- Closely collaborate with other care teams within Polara to coordinate and meet client needs (Inpatient Hospital, Crisis Stabilization Unit, Outpatient Services, Primary Care)
- Facilitate Adult Recovery Team Meetings (ARTs)
- Participates as a team member and attends scheduled staffings and supervision.
- Partners with client and team to identify treatment goals
- Maintains clinical record per agency policy and procedure.
- Attends all team meetings, case reviews, staff meetings, supervisory sessions, and in-service trainings
- Management of all aspects of treatment, including referrals to additional resources when appropriate.
- Performs other duties as assigned.
Qualifications
- Minimum Bachelor's degree in related field OR combination of education and experience to equal 4 years in behavioral health will be considered.
- Must possess valid driver's license
- Ability to assess situations and demonstrate critical thinking and problem solving abilities
- Understanding of mental health and impact on functioning
- Understanding of trauma informed care
- Experience working with individuals in crisis situations and/or in need of intensive support services
