Job Overview: As a Community Support Specialist, you will help individuals adapt to their communities by creating personalized plans tailored to their specific needs. Utilizing evidence-based techniques, you will collaborate with other professionals to provide comprehensive mental health services. Key Responsibilities:

* Provide community-based mental health services, including assessment, diagnosis, treatment planning, and intervention implementation for children, adolescents, and adults.

* Collaborate with clients, families, and stakeholders to develop and implement individualized treatment plans aligned with client goals.

* Utilize a strengths-based approach, incorporating evidence-based practices such as Motivational Interviewing, Positive Reinforcement Techniques, Trauma-Informed Strategies, and skills in CBT and DBT.

* Implement behavioral interventions, providing motivation and support to clients.

* Coordinate care with primary care physicians, psychiatrists, schools, and community organizations.

* Maintain accurate records of client interactions, treatments, and progress per organizational policies.

* Participate in staff development activities, enhancing skills related to mental health services.

* Contribute to quality improvement initiatives through data collection, analysis, and reporting.

* Work collaboratively as part of a multi-disciplinary team to ensure effective coordination of care.

* Build and maintain relationships with key stakeholders involved in clients' care, including parents, teachers, and counselors.

* Prepare for and facilitate PSR group sessions relevant to client treatment plans.

* Perform other duties as assigned by supervisors or as needed by the department.

Knowledge, Skills, and Abilities:

* Thorough knowledge of case management and rehabilitation methods related to mental health and substance use disorders.

* Understanding of challenges faced by individuals with mental health/substance use disorders and available community resources.

* Familiarity with legal requirements and procedures in treating mental health and substance use issues.

* Strong communication skills to effectively produce clear reports and problem-solve.

* Self-motivated with the ability to meet complex needs of clients and families.

* Capable of multitasking and coordinating services while maintaining communication across departments.

* Ability to establish effective relationships with clients and their families.

Experience and Education Qualifications:

* Bachelor's degree in Psychology, Social Work, or a related human services field; or

* Bachelor's degree in an unrelated field with two years of related work experience; or

* A combination of higher education and two years of related work experience; or

* Associate's degree in Behavioral Health Support; or

* Four years of qualifying experience involving direct service delivery to individuals with mental health disorders, substance use disorders, or developmental disabilities.

Supervisory Requirements:

* None

Employment Requirements:

* Successful completion of background checks (criminal record, driving record, abuse/neglect, and fingerprint checks).

* Completion of New Hire Orientation and adherence to training requirements including annual Relias training.

* Valid driver's license and acceptable driving record with current auto insurance.

* Must be at least 21 years of age to operate a company-owned vehicle; 23 years of age for a 15-passenger van.

* Reliable means of transportation for client transport.

Physical Requirements:

* ADA Consideration: Sedentary work involving exerting up to 10 pounds of force occasionally and negligible force frequently.

* Repetitive movements of hands and arms for typing/writing throughout the work shift.

* Sedentary work typically involves sitting but may include brief periods of walking or standing as needed.
