Etablished in 1912, the Jewish Home of CNY evolved from a small rooming house in downtown Syracuse to become the 36-acre campus in DeWitt known as Menorah Park. Must be a licensed CNA in NYS.Broad Function:The Activities Leader/Director shall independently provide a program of varied, engaging, creative, entertaining activities tailored to meet individual needs for the purpose of physical, emotional, intellectual, educational, and spiritual development of the Registrants at the Adult Day Health program.Major Responsibilities:The Activities Leader/Director shall create and implement a comprehensive Activities Program as delineated above.Duties:1. Planning high quality, creative monthly activity calendar to include varied activities in the realms of entertainment, community outings, physical exercise, creative arts, cognitive programs, spiritual programs, etc.
Creating monthly activity calendar on PC.Post daily activities on marker board in program area.Adhere to daily activities schedule to best of ability.Assess Registrants activities interests upon admission and every 6 months thereafter.Participate in the care planning process including creating individualized activities care plans for each resident, completing documentation as required by DOH, and attending Registrant care plan meetings.Maintain daily statistics on Registrant participation. Maintain and purchase supplies and equipment within budget. Keep program area and supply closets neat and orderly.Party and event planning at ADHC.Decorate program area for parties/celebrations/holidays/seasons in a timely manner.Willingness to operate Menorah Park van on community outings.Communicate concerns re: Registrants with other ADHC staff.Assist RN & SWK with coordinating transport as needed.Monthly Quality Assurance.Acts in a professional manner with vendors, Registrants, families, and other staff at all times.Adheres to HIPAA standards.Provide hand-over-hand assistance to impaired Registrants as needed.Assist Registrants with feeding at mealtimes as needed. Attends and facilitates monthly Registrant Council meetings and follows up as needed on new business . Possess knowledge of fire safety procedures and be able to carry out procedures during drills and emergencies.Responsible for photographing special events. Education and Experience:Minimum Associate's Degree preferred. High School graduate with 2 years experience in providing therapeutic activities will be considered. Good written, verbal, computer, and organizational skills. Ability to maintain good relationships with peers, supervisor, Registrants, and families. Possess CDL license or willingness to obtain. Must be creative, energetic, and able to think outside of the box.
