Rochester, NY [On-Site]FULL-TIME [Nonexempt] : 40 hours per week Coordinated Care Services Inc (CCSI) seeks a talented Adult Single Point of Access Program Associate to contribute their expertise in providing essential business services to our provider organizations and customers. The Adult Single Point of Access (A-SPOA) Program Associate is responsible for entering, triaging, and routing referrals for A-SPOA. This includes communication with partnering agencies, coordination and facilitation of complex case review sessions and partnering with existing Department staff to build the A-SPOA committee in alignment with building expertise in the Broome County adult system of care.
