Advanced Care Practitioner (ACP) Under the general guidance of the Chief Medical Officer and in accordance with the Board of Medical Examiners directives, the ACP will follow Standards of Care - Best Practice Guidelines in the clinic and/or hospital setting.
- Evaluates, diagnoses, and treats diseases and adverse health conditions.
- Order and evaluate diagnostic testing. Identify proper diagnosis and create appropriate plan of care.
- Patient education regarding treatments, procedures, tests and their subsequent results.
ACP/Hospitalist In addition to the role and function of ACP above, the ACP/Hospitalist will:
- Deliver primary and specialty patient care with a high degree of expertise.
- Formulate an evidence-based care plan based on assessment, examination, and selected diagnostic procedures within their scope of practice.
- Collaborate with physicians and other members of the healthcare team.
- Work to achieve the highest level of satisfaction of coordinated clinical management from patients and families.
- Provide Hospitalist functions that center around population management of inpatients by directing, coordinating and supervising patient care activities.
- Provide diabetes education and wound care.
- Provide education to patients, families, and clinical staff.
- Work in conjunction with Inpatient Services Manager and staff to ensure optimal care and outcomes for patients.
- Participate in and provide recommendation regarding best practice to Hospital Engagement Network Teams.
- Provide additional services as directed by the CMO.
ACP/Walk-In & Primary Care Clinic In addition to the role and function of ACP and the ACP/Walk-In Clinic will:
- Provide timely and efficient medical services to all patients who present to the Clarke County Family Medicine Walk-In Clinic during the following hours:
- Monday - Friday 07:00 - 19:00
- Saturday and Sunday 09:00 - 17:00
- Coverage for Walk-In Clinic hours will be provided in schedule format for the Clinic Manager and approved by Chief Medical Officer
QUALIFICATIONS:
- Certified Physician's Assistant (P.A.-C) or Advanced Registered Nurse Practitioner (ARNP) or Doctoral Nurse Practitioner (DNP).
- Graduate of an accredited Family Nurse Practitioner Program and hold a current license.
- Certification in specialty areas or demonstrate active progress toward attainment of certification.
- Current licensure as P.A.-C in state of Iowa or current licensure as RN and ARNP in state of Iowa
- Education in management and care of chronic and acute wounds and ostomy care.
- Current prescriptive authority in the State of Iowa including DEA Registration.
- Current BCLS and ACLS certifications.
- Must have a desire to serve patients and be self-motivated
- Strong written and verbal communication skills
- Strong interpersonal communication skills for working with patients and clinical team members.
- Ability to accept responsibility and account for own actions.
