Job Description:
Advanced Care Provider (ACP) is a Certified Physician's Assistant (P.A. -C) or an Advanced Registered Nurse Practitioner (ARNP) or a Doctoral Nurse Practitioner (DNP) who delivers primary and specialty patient care with a high degree of expertise.Professional services include formulating an evidence-based care plan based on assessment, examination, and selected diagnostic procedures within their scope of practice.There are 2 roles which fall under this job description:Advanced Care Practitioner in RHC.ACPUnder the general guidance of the RHC Medical Director and in accordance with the Board of Medical Examiners directives, the ACP will follow Standards of Care - Best Practice Guidelines in the clinic and/or hospital setting.
  • Evaluates, diagnoses, and treats diseases and adverse health conditions.
  • Order and evaluate diagnostic testing. Identify proper diagnosis and create appropriate plan of care.
  • Patient education regarding treatments, procedures, tests and their subsequent results
ACP/ Rural Health ClinicThe ACP/Rural Health Clinic will:
  • Provide timely and efficient medical services to all patients who present to the Clarke County Family Medicine Convenient Care Clinic.
    • Coverage for Convenient Care Clinic hours will be provided in schedule approved by RHC Medical Director
Relationship Building & Patient Education
  • Coordinates care with outside sources including Lab, Diagnostic Imaging, Private Health Care Providers, Social Workers, and Public Health.
  • Balances team and individual responsibilities, objective and open to others' views, gives and welcomes feedback, contributes to positive team goals, and puts success of team above own interest.
  • Develops and maintains curriculum for inpatient and outpatient diabetes education programs.
  • Utilizes effective education techniques for age related care of pediatric, adolescent, and geriatric patients.
  • Collaborates and works effectively with interdisciplinary team members and community health agencies.
  • Develops and maintains effective physician communication and collaborative relationships.
  • Consistently demonstrates positive interdisciplinary communication and promotes positive work environment behaviors.
  • Demonstrate respect and regard for the dignity of all patients, families, visitors, and fellow employees to ensure a professional, responsible, and courteous environment.
Clinical Excellence
  • Performs procedures specific to the service lines competently.
  • Acts as a resource person for other staff and personnel with which clients have contact.
  • Demonstrates the ability to consistently meet the department work schedule.
  • Maintains current knowledge of emergency medical procedures and responds appropriately
  • Assists with and provides simple and complex procedures appropriate to the scope of practice.
  • Assists in maintaining a clean and orderly environment.
  • Demonstrates initiative to improve quality and customer service by striving to exceed customer expectations.
  • Demonstrates the knowledge that patient and workplace safety is a priority by preventing errors, accidents and infections and reporting potential or actual concerns immediately.
  • Develops, implements and monitors evidence based order sets, protocols, policies, procedures, documentation and patient monitoring standards.
  • Practices in accordance with current ADA Clinical Practice Guidelines
  • Collects data, monitors and reports patient outcomes in relation to national benchmarks.
  • Incorporate quality improvement data and/or patient satisfaction data in clinic goals.
  • Assists in unit specific duties (i.e. management responsibilities, as appropriate to the assigned area.
  • Provides education to clinical staff when needed.
  • Attends continuing medical education programs to maintain licensure and Board Certifications in area of primary specialty.
  • Reports problems or concerns to the RHC/Outpatient Medical Director on a timely basis.
Basic Clarke County Hospital (CCH) Performance Criteria
  • Demonstrates Clarke County Hospital Standards of Behaviors as well as adheres to hospital policies and procedures and safety guidelines.
  • Demonstrates ability to meet business needs of department with regular, reliable attendance.
  • Employee maintains current licenses and/or certifications required for the position.
  • Practices and reflects knowledge of HIPAA, DIA, DNV, OSHA and other federal/state regulatory agencies guiding healthcare.
  • Completes all annual education and competency requirements within the calendar year.
  • Is knowledgeable of hospital and department compliance requirements for federally funded healthcare programs (e.g. Medicare and Medicaid) regarding fraud, waste and abuse. Brings any questions or concerns regarding compliance to the immediate attention of hospital administrative staff.Takes appropriate action on concerns reported by department staff related to compliance.
Other RequirementsAs an employee of Clarke County Hospital, you may be required to assist in response activities for disasters, emergencies and pandemics which may occur outside of normal working hours.Qualifications:Education:
  • Certified Physician's Assistant (P.A.-C) or
  • Advanced Registered Nurse Practitioner (ARNP) or
  • Doctoral Nurse Practitioner (DNP).
  • Graduate of an accredited Nurse Practitioner Program.
  • Certification in specialty areas or demonstrate active progress toward attainment of certification.
  • Current licensure as P.A.-C in state of Iowa
  • Current licensure as RN and ARNP in state of Iowa
  • Education in management and care of chronic and acute wounds and ostomy care.
Experience:
  • Experience in management and care of chronic and acute wounds and ostomy care.
  • EPIC charting.
License(s)/Certification(s):
  • Valid driver's license when driving any vehicle for work-related reasons.
  • Current, active, and unrestricted Physician's Assistant Board Certification.
  • Current, active, and unrestricted RN and ARNP license in the State of Iowa with appropriate certifications as necessary by licensing boards.
  • Current prescriptive authority in the State of Iowa including DEA Registration.
  • Current BCLS and ACLS certifications.
