This position requires extensive travel within greater Monmouth & Ocean County New Jersey, as well as the ability to function independently. Care will be provided direct in a patient's home, assisted living facilities, group homes, and/or nursing homes.

Position Highlights:

  • Clinical Aptitude: Prior experience in providing home care highly preferred. This position requires a high degree of independence/autonomy.
  • Full-time; Flexible Scheduling.
    • 40 hours per week, Monday – Friday, flexible day and/or afternoon scheduling options.
    • Clinicians schedule their own patients daily - highly flexible.
    • No on-call requirements.
  • Average patient load = Minimum of 8 to 10 patients per day.
  • Teamwork; Evidence-based Medicine.
  • Autonomy to practice at the peak of your license.
  • Supported to grow your practice and patient panel.
  • Must be comfortable working independently (but always have Physician support available via phone).
  • Must have consistently reliable transportation.

Primary Responsibilities:

A Home Care Physician Assistant or Nurse Practitioners conducts health assessments on enrolled Medicare Advantage members in their personal homes, assisted living facilities, group homes, and/or nursing homes – many of whom are elderly, physically/mentally disabled, and/or homebound. A Home Care practitioner will complete visits on members within a specified geographic area of responsibility. The nature of the Home Care visit can vary and could include performing an annual health assessment, a post discharge visit, or visiting more complex members routine care (i.e., every 4-6 weeks). Practitioners can structure their schedules and visits to coordinate care in the same geographic area(s) daily.

General Responsibilities:

  • Conduct assessments on health plan members. Average visits vary from 30 - 60 minutes in length, based on complexity.
  • Home Care assessments includes:
    • Past medical history.
    • Review of systems.
    • Physical examination.
    • Medication review.
    • Depression screening.
    • Responsible for checking vitals, conducting a physical exam that includes monofilament test, urine dipstick, and foot exam (as appropriate).
    • Identify diagnoses to be used in care management and active medical management in the furtherance of treatment.
    • Formulate a list of current and past medical conditions using clinical knowledge and judgment and the findings of your assessment.
    • Communicate findings in your assessment that will be used to inform members PCP of potential gaps in care.
    • Identify needs and opportunities to generate clinical referrals to Care Improvement Plus and / or the primary care physician.
    • Educate members on topics such as disease process, medication, and compliance.
    • Compliance with all HIPAA regulations and maintain security of protected health information (PHI).

Required Qualifications:

  • Master's degree as a Physician Assistant of Nurse Practitioner from an accredited program
  • Certified as a Physician Assistant through the NCCPA or as a Family Nurse Practitioner through the ANCC and/or AANP
  • Active and unrestricted Physician Assistant or Nurse Practitioner license in the State of New Jersey.
  • Active New Jersey CDS license (or ability to obtain prior to start)
  • Active and unrestricted DEA License (or ability to obtain prior to start)
  • Must be CPR certified (or willing to obtain prior to start)
  • Prior/current clinical home care experience
  • Minimum of 1 to 3 years post graduate clinical experience highly preferred (i.e., prior experience in gerontology, cardiology, internal medicine, or endocrinology)
  • Must be willing to travel within the greater Monmouth and Ocean counties areas in New Jersey
  • Must have reliable transportation
  • Must be a highly effective communicator, organized, and able to practice independently

New Jersey Residents Only: The salary range for this role is $109,513 to $147,146 per year. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
