Make Bristol Health your choice.Job Details:Bristol Health Medical Group has moonlighting opportunities for advanced practice practitioners to join the Department of In-patient Medicine at Bristol Hospital.We are seeking compassionate, mission-focused, providers who want to make a difference and provide quality care to patients in our communities. You will be part of a collegial and collaborative team of providers who are dedicated to Bristol Hospital's goal of being one of the best community hospitals in Connecticut.The Hospitalist service provides 24/7/365 inpatient hospital coverage for all patients admitted to the medical and surgical floors.: Clinical Responsibilities:For patients under their management, the APRN/PA shall conduct initial patient assessments and admission procedures; formulate and implement a treatment plan; schedule and review clinical and diagnostic tests as medically necessary for each patient; provide timely and regular reports to primary care physicians and family members regarding all aspects of patients' medical conditions and course of treatments; conduct multiple rounds daily as necessary for the acuity of patients' conditions; coordinate and integrate specialty and subspecialty consultations; coordinate patient transfers to and from the intensive care unit, medical/surgical unit, skilled nursing facility or home; conduct timely and proactive discharge planning to include ensuring appropriate follow-up with the primary care physician/specialty physician has been arranged.The APRN/PA shall coordinate with the case management department of the Hospital as appropriate for each patient under their care for timely and proactive discharge planning. APRN/PA shall inform Hospital and/or primary care physicians of any problems identified after a patient is discharged, to include notification of any abnormal pending diagnostic reports. Along with other members of the Hospitalist team, provide coverage 24 hours a day, 7 days a week, in accordance with scheduling as approved by administration, onsite, responding within time frames established by the medical staff for emergent, urgent and routine requests.APRN/PA shall be available for and respond to provide admission, management and consultation of medical/surgical patients and other hospital affiliated patients including but not limited to psychiatric patients, as requested.APRN/PA shall maintain an effective communication process to interface with referring primary care physicians, attending physicians, consultants, nursing, patients and family members.APRN/PA shall accurately and completely document all patient encounters in patient medical records and encounter forms consistent with evaluation and management documentation guidelines.Perform any other duties reasonably related to this Agreement as requested by Hospital Administration or their designee.Administrative Responsibilities:Participate and cooperate in the completion of the monthly hospitalists' team schedule.Participate in compliance, in-service and other administrative programs related to the responsibilities of this Service.Cooperate with Hospital Administration or its designees in all matters related to the recovery of reimbursement for professional services performed under this Agreement including, but not limited to, the enrollment in all insurance plans designated by Hospital and reassignment of all reimbursements for hospitalist services to the Hospital.Assist in the development and implementation of clinical pathways, protocols, core measure compliance and other clinical effectiveness projects at the request of Hospital Administration or their designee.Comply with all Medical Staff Bylaws and rules and regulations and Hospital policies and procedures as currently exist or are hereinafter amended.Participate and comply with BHMSG and Hospital compliance programs and activities, including participation in any compliance education or programs deemed necessary or appropriate by BHMSG or Hospital including but not limited to billing and coding compliance programs.Comply with all BHMSG reimbursement requirements including the timely completion of billing slips and accurate coding of services.Serve on BHMSG, Hospital and Medical Staff committees as requested by the Employer.Attend and participate in regularly scheduled meetings with other professional members of BHMSG and the Hospital.Good Citizenship Responsibilities:Maintain excellent working relationships with, and participate in the implementation and improvement of systems to provide excellent service to patients, physicians, colleagues, and Hospital staff. Participate in the resolution of patient and provider complaints and enhancement of patient and provider satisfaction as measured by periodic surveys. Maintain excellent working relationships and communication with the other members of the Hospitalist practice, support personnel provided to the Hospitalist practice, Hospital administration, and Medical Staff leadership. Participate as requested by the Hospitalist practice Medical Director, in periodic team meetings, e-mail and/or other communication methods to foster effective working relationships and reduce variation in practice patterns.Complete and maintain adequate and proper medical records with respect to all patients examined or treated in accordance with all legal, accreditation, regulatory and third party payor requirements.Attend periodic meetings to review the effectiveness and performance of the Hospitalist program and its providers.Participate in community benefit programs or other activities as requested. QualificationsQualifications:
  • Applicant must be a graduate of a Physician Assistant training program accredited by the ARC-PA, or one of its predecessor agencies (CAHEA or CAAHEP) or Certification by the American Nurses Credentialing Center or American Academy of Nurse Practitioners
  • Valid Connecticut medical license, Connecticut Controlled Substance Registration and Federal Drug Enforcement Agency (DEA) Certification.
  • ACLS certification
