We are looking for an experienced Advanced Practice Provider (APP) to join our Geriatrics team. In this role, you will work closely with physicians and healthcare professionals to provide thorough and compassionate care to geriatric patients. Your responsibilities will include conducting assessments, developing treatment plans, and managing chronic illnesses. We welcome applicants with experience in nursing homes, post-acute, or long-term care settings, and we are open to new graduates with relevant experience.

  • Conduct thorough evaluations and assessments of geriatric patients, including physical exams and health history reviews.
  • Develop and implement individualized care plans based on patient needs and evidence-based practices.
  • Manage and monitor chronic conditions such as diabetes, hypertension, and dementia, adjusting treatment as necessary.
  • Provide education and support to patients and their families regarding health management and lifestyle modifications.
  • Collaborate with interdisciplinary teams to coordinate care and optimize health outcomes.
  • Participate in quality improvement initiatives and contribute to the development of best practices in geriatric care.
  • Stay current with advancements in geriatric medicine and relevant healthcare regulations.
  • Masters degree in nursing.
  • Valid state licensure as a Nurse Practitioner (NP).
  • Certification in Geriatrics or a relevant specialty is preferred.
  • APP Experience in nursing homes, post-acute, or long-term care settings is preferred, but we are open to new graduates with relevant nursing experience.
  • Strong communication skills and the ability to connect with older adults and their families.
  • A commitment to providing patient-centered care.
