Nemours Children's Health, Jacksonville Clinic is hiring a Neurology Advanced Practice Provider. The Advanced Practice Provider (APP) will provide direct and indirect care to patients and their families in the Nemours Neurology Division and at Wolfson Childrens Hospital, Epilepsy Monitoring Unit. Responsibilities include new inpatient and outpatient consults for programmatic growth, as well as outpatient follow up visits for established patients. The APP will also manage the health care of those patient which he/she has been educated. In addition, to outpatient consults, the APP may provide subsequent outpatient follow up visits for epilepsy patients following hospital discharge and complex care coordination for medically complex patients as patient volume requires. Must be capable of self-directed independent and collaborative practice. Strong communication and inter- professional skills are required as this role is a critical liaison between medical teams, nursing teams, and families. Essential Functions Practices collaboratively with physicians and other health care providers to provide high quality healthcare to children. Performs appropriate assessment of patient/family upon referral/admission. Performs ongoing assessment and evaluation of patient progress and revision of interdisciplinary plan of care. May use prescriptive authority if appropriately credentialed to do so. Collaborates inter-professionally as needed. Identifies patient/family need for further resources and initiates appropriate referrals. Assists the Staff Nurse in formulating the Nursing Care Plan and setting priorities based on the needs of patient and family. Documents patient care and progress on patient's chart. Coordinates Nursing Plan for Discharge in collaboration with the Staff Nurse and other members of the health care team. Provides patient/family teaching based on identification of learning needs. Evaluates compliance with the prescribed treatment regimen through telephone calls, family/patient assessments and outpatient visits as needed. Acts as a patient's advocate; acts as a role model to other members of the Nursing Department to foster professional development; acts as a liaison between families and members of the health care team. Utilizes advanced knowledge and specialized skill in nursing practice in accordance with the Nurse Practice Act, hospital policy and protocol. Identifies educational needs of nursing staff regarding the care of patients in the specialty area. Serves as an educational resource for unit and the Organization. Collaborates with nursing management in the monitoring and evaluation of the quality and appropriateness of patient care within the specialty area. Fosters a spirit of inquiry, identifies nursing problems, and participates in research endeavors. Disseminates nursing research findings and incorporates these findings into nursing practices for patient care within the specialty area. Serves as a consultant within specialty area for colleagues both intra and extra departmentally and in the community. Maintains a current knowledge base through participation in appropriate professional activities and organizations. Demonstrates effective communication skills with health care team and patient, family, and members of the health care team. Responsible for participation in departmental and hospital programs for Quality Assessment and Improvement, identifying opportunities to improve services, making recommendations, and implementing actions as appropriate and consistent with the goals of the Nemours Foundation. Requirements: APRN: Current unrestricted FL APRN license and FL DEA; Pediatric Nurse Practitioner Certification via National Certification Center (Pediatric Nurse Practitioner-Acute Care Strongly Preferred); AHA BLS and PALS upon hire. PA-C: Current unrestricted FL Physician Assistant License and FL DEA; National Certification from the National Commission on Certification of Physician Assistants, AHA BLS & PALS upon hire.
