Location: Chicago, IL Hospital: RUSH University Medical Center - Virtual First Clinic Department: Internal Medicine, Division of General Internal Medicine Work Type: Full Time (Total FTE between 0.9 and 1.0) Shift: Shift 1 *Advanced Practice Provider experience in Primary Care is required. *Applicants must hold an AG-PCNP, FNP, or PA certification. Summary: Rush University Medical Center, a nationally recognized clinical and academic institution and the teaching hospital for Rush Medical and Nursing Colleges, located in downtown Chicago, is seeking an Advance Practice Provider to join our growing Primary Care team in the new Rush Primary Virtual First Clinic launched in January 2024. We are seeking a passionate Advanced Practice Provider to help build the primary care clinic of the future. Virtual First Clinic will aim to recreate the experience all of us hope to have in healthcare by simplifying accessibility to quality healthcare and strengthening the provider-patient relationship. In this model, Rush faculty will deliver convenient and compassionate care for each patient using a technology-augmented, team-based approach. Our collective mission is to improve population health, enhance the patient experience, reduce unnecessary healthcare costs, improve clinical staff experience, and decrease health inequities. The ideal team member is process-oriented, compassionate/empathetic, innovative, and has experience working in team-based models. Qualified applicants must be licensed physician assistants (PA) or advanced practice registered nurses (APRN) in IL, and possess a demonstrated history of professionalism, team building, and providing excellent clinical care.Advanced Practice Provider experience in primary care is required. The Advanced Practice Provider (APP) who assumes this role will function in collaboration with the physician faculty. The APP will support the physicians patient panel, within the scope of practice granted in accordance with the RUMC medical staff bylaws. The APP will be expected to see patients independently where applicable. Virtual First Clinic ModelTeam-Based Care: Together the physician, advanced practice provider, medical assistant (MA), social worker (SW), registered nurse (RN), and pharmacist (PharmD) will manage a patient panel. This role will combine traditional outpatient clinic responsibilities with remote patient monitoring (RPM) and chronic care management (CCM) • Technology Augmentation: The clinic will employ cutting-edge technology to help augment staff workflow and deliver quality and sustainable care for patients. • Clinic Operations: Virtual First Care clinicians will have a schedule consisting of three (3) virtual workdays per week and two (2) in-person care days a week in the clinic. The hybrid work environment will be standardized with operational with optimized clinical tools and guidelines. The Advanced Practice Provider functions as a member of the health care team to provide high quality, comprehensive acute and/or routine medical/surgical care of Rush University Medical Center patients within the scope of practice granted in accordance with the RUMC medical staff bylaws. Promotes collaboration between multiple disciplines to ensure cost effective and quality patient care. Successful candidates will also meet all other state and federal regulatory practice and accreditation standards. Exemplifies the Rush mission, vision and values and acts in accordance with the policies, procedures, guidelines of Rush, including the Rush University Medical Group Customer Service standards. Rush has over 400 APPs across the enterprise that provide comprehensive patient care each day autonomously and in collaboration with other members of the health care team. With a Director of Advanced Practice and 30 Lead APPs across the system, Rush's vision is to have every APP practicing at full scope and top of license. APPs at Rush are highly competent, well-respected members of the healthcare team that contribute to the excellent quality care that Rush patients receive each day. The individual who holds this position will exemplify the Rush mission, vision and values and act in accordance with the policies, procedures, guidelines of Rush, including the Rush University Medical Group Customer Service standards. Successful candidates will also meet all other state and federal regulatory practice and accreditation standards. Responsibilities: CLINICAL 1. Completes and documents an accurate history and physical examination of patients. 2. Conducts comprehensive or episodic health assessments and develops patient care plans. 3. Orders and interprets tests, treatments, and therapies within the scope of practice 4. Prescribes medications, including controlled substances to the extent delegated including non-pharmacologic treatment modalities. 5. Orders, interprets, and performs laboratory and other diagnostic tests and procedures. 6. Provides clear, concise, and accurate information in the electronic medical record. 7. Initiate and perform consultations in accordance with Rush rules and regulations. 8. Make appropriate referrals to other health care professionals and/or agencies. 9. Provides direct patient care in assigned areas, which may be in a hospital or ambulatory setting. PROFESSIONAL 10. Works collaboratively with his/her collaborating/supervising physician, department, and other members of the healthcare team in an effective and comprehensive manner. 11. Acts as an advocate for patients, families, coworkers, and the physician assistant profession. 12. Participates in all required provider activities as outlined in the RUMC Medical Staff Bylaws. 13. Integrates evidence based knowledge and clinical expertise in decision making. 14. Demonstrates competency in all medical and surgical procedures and skill considered essential in their area of practice. 15. Maintains board certification, state licensure, and continuing medical education as outlined by hospital, state, and federal policy. LEADERSHIP 16. Supports the organization's overall strategic plan and direction. 17. Serves as a leader of the healthcare team to provide integrated delivery of patient care service 18. Actively participates in ongoing and systematic quality improvement efforts which focus on patient care delivery and supports processes that provide optimal patient outcomes. 19. Demonstrates accountability for own professional practice and improvement. Including ongoing self-assessment of core competencies and yearly goals. 20. Develops and leads implementation of department goals by collaborating with department staff, students, and physicians. 21. Serves as a role model and thought leader for their own profession including practice analysis and advancement to achieve optimal patient outcomes. 22. Manages resources effectively thereby considering the institution and practice's fiscal responsibilities. 23. Demonstrates an understanding of patient documentation, compliance, and billing best practices. EDUCATION 24. Provides relevant and comprehensive patient education to patients, families and significant others. 25. Fosters growth and development of students and other healthcare professionals. 26. Continuously assesses their personal capabilities and limitations, striving always to approve their medical practice. 27. Use their knowledge and expertise to contribute to an improved community. Other information: Required Job Qualifications: • Current State of Illinois Physician Assistant Licensure. • Current Certification from National Certification Commission for Physician Assistants (NCCPA) or other agency recognized by the American Academy of PAs (AAPA). • Degree from an ARC-PA (or predecessor) accredited PA Program. • BLS Certification. • Demonstrates competency in all areas of APP practice including: medical knowledge, interpersonal and communication skills, patient care, professionalism, practice based learning and improvement, and systems based practice. • Skill in preparing and maintaining electronic and paper medical records • Knowledge of current and emerging trends in technologies, techniques, issues, and approaches in area of expertise. • Strong work ethic, interpersonal and communication skills. • Ability to work autonomously and collaboratively with collaborating physician, department, and other members of the healthcare team in an effective and comprehensive manner.
