Job Title: Advanced Practice Providers (NP/PA)
Pay Range - $60-$70+/hr (depending on experience)
Location: Travel Required throughout Albany, NY
- Albany and Rensselaer County
- Albany and Columbia County
Shift: Monday-Friday from 8am-5pm
Contract Duration- 3-4 months
General Overview:
We are seeking dedicated and compassionate Advanced Practice Providers both Nurse Practitioners and Physicians Assistants (NP/PA) to join our Healthcare Team. As an Advanced Practice Provider (NP/PA) you'll provide personalized, high-impact care to our highest-risk members in a facility setting and within patient homes. As an APP, you'll be part of an interdisciplinary team focused on improving patient outcomes through comprehensive, integrated care. Essential Functions:
- Perform and complete a comprehensive geriatric assessment on each resident
- Provides primary, preventative and urgent/acute care at a SNF or within Patient Homes
- Develops and communicates a problem list and medical treatment plan for each resident
- Develops a contingency plan for high-risk conditions
- Reaches out to responsible party after each visit to establish lines of communication and keep informed
- Manage acute and chronic medical/behavioral conditions
- Ensure accurate documentation, including ICD-10 coding
- Collaborate with an interdisciplinary care team
- Lead advanced illness and care planning conversations
- Educate and counsel patients and caregivers
Job Requirements
- Master's or Doctorate degree in Nursing from an accredited program.
- Current, unrestricted Nurse Practitioner or Physician Assistant license and certification.
- Active DEA license/prescriptive authority
- 1+ year of clinical experience (preferably in long-term care)
- Experience with complex behavioral, social, or chronic care needs
- Knowledge of geriatrics, chronic illness, and end-of-life care
- Ability to work independently and collaboratively in a multidisciplinary team environment.
- Commitment to providing compassionate care to Seniors and their families.
- Strong clinical assessment, diagnostic, and treatment skills.
