Come join a team of caring staff who are making a difference in the lives of our clients/consumers.Position Requirements The Goal of ADvantage is to keep members as independent as possible in their home for as long as possible. A Case Manager coordinates those services to assure they are beneficial and appropriate.Case Managers collaborate with members by assessing, facilitating, planning, advocating and monitoring health and social needs, thus enabling the member to remain in the community. Administer case management within the scope of practice of the ADvantage Medicaid Waiver program.Personal care aides work mostly with the elderly who need help with daily tasks and chores, working in places such as residential care facilities and member homes. Personal care aides generally are responsible for light cleaning, cooking, running errands, and doing laundry, as well as assisting members with bathing, showering, grooming, and other personal hygiene tasks.Additional Requirements:
  • BA or BS in social sciences, health or any closely related field and two (2) years professional work experience in a position utilizing case management. And or Diploma in Nursing with current Oklahoma Registered Nurse registration and at least 4 years professional work experience.
  • Certified Case Manager for the ADvantage Program through Department of Human Services.
  • Oklahoma driver License
  • Excellent verbal, interpersonal and written communication skills, as well as problem solving skills. Ability to organize and manage a caseload in accordance with agency standards and in compliance with policies and procedures of contracted programs. Ability to relate well with aged and disabled persons in a variety of settings in the home and community. Knowledge of community resources and aging issues. Ability to meet deadlines, to work independently and interdependently as part of the planning team.
  • Good hearing and visual capacity. 70% of the work is performed while sitting. Dependable transportation.
