The Family Advocate advocates and promotes Metrocare Services from a primary caregivers point of view. This position serves as part of the interdisciplinary team in identifying and addressing the needs of the primary caregivers as it relates to the consumer. ESSENTIAL DUTIES AND RESPONSIBILITIES: The essential functions listed here are representative of those that must be met to successfully perform the job. Provides peer mentoring and supports to the primary caregivers. Introduces families to the treatment process. Models self- advocacy skills. Provides information, referral and non-clinical skills training. Assists in the identification of natural/non-traditional and community support systems. Documents the provision of family partner services including both face-to-face and non-face-to-face activities. Collaborates with clients to develop an individualized care plan that is grounded in the family members perspectives and includes items that reflect the familys values and preferences. Works in tandem with team partner the Intensive Case Manager to achieve team goals. Assists the family in selecting individuals to be on the families team; these may include both formal and information supports. Participates in interdisciplinary team staffing. Presents psychosocial assessment finds. Updates team on progress or lack of progress in reaching agreed upon goals. Provides relevant information that might affect treatment. Follows-up on assigned consumers who miss scheduled appointments. Documents attempts to contact consumer. Reports repeated unsuccessful efforts to contact the treatment team. Follows-up within 24 hours on assigned consumers who access emergency services. Reports status-disposition to treatment team. 100% compliance expected. Performs other duties as assigned. QUALIFICATIONS EDUCATION AND EXPERIENCE: High School Diploma or equivalent MATHEMATICAL SKILLS: Basic math skills required. REASONING ABILITY: Ability to apply common sense understanding to carry out simple one or two-step instructions. Ability to deal with standardized situations with only occasional or no variables. COMPUTER SKILLS: Use computer, printer, and software programs necessary to the position (i.e., Word, Excel, Outlook, and PowerPoint). Ability to utilize Internet for resources. CERTIFICATIONS, LICENSES, REGISTRATIONS: Current State of Texas Driver License or if you live in another state, must be currently licensed in that state. If licensed in another state, must obtain Texas Driver License within three (3) months of employment. Liability insurance required if employee will operate personal vehicle on Center property or for Center business. Must be insurable by Centers liability carrier if employee operates a Center vehicle or drives personal car on Center business. Must have an acceptable driving record.
