Job Overview:Schedule: 9am-5pm Monday to Friday or Tuesday to Saturday, with one late day 12pm-8pmType: Full time, RegularPay: $60,000 annual salaryLocation: Fully onsite50-60% in the Lower East Side40%-50% field work traveling through the 5 boroughs of NYCJob Summary:As Aftercare Case Manager you will provide outreach service support to clients who have been diverted from the shelter system and are living in the community. Your mission will be to reduce the recidivism rate of families returning to the shelter. You will be responsible for providing services and referrals such as assessments, counseling, service planning, entitlements, medical, early childhood, educational, substance abuse, employment, and mental health services. In coordination with other Transitional & Supportive Housing staff, you will identify possible barriers to permanency and secure the resources necessary to promote stability.Qualifications:Bachelor's degree required; Master's degree in social work preferred3-5 years of case management experience required with at least 2 years of experience in a social service agencyStrong engagement, interpersonal, and communication skills required with the ability to facilitate group workshops and conduct one-on-one individual meetings with clientsStrong computer skills, including working knowledge of Microsoft Office and AWARDS, and CARESStrong research skills and knowledge about social service offerings in NYC and Westchester CountyExperience working with the homeless population and an understanding of diverse populations requiredExcellent organizational, analytical, and problem-solving skills required to work independently while meeting funder and program requirements and deadlinesAbility to be flexible while maintaining prioritiesBilingual candidates (English and Spanish) encouraged to applyResponsibilities:Conduct in-home visits on a regularly scheduled basis with clients who have left shelter within past 2 years to provide supportive counseling and to review progress towards housing retentionProvide advocacy, information and referral services for housing, entitlements, employment, vocational training, and educationMaintain tracking spreadsheet and accurate files for all aftercare clients on a daily and weekly basisWrite clear and concise case notes in AWARDS after every client interaction Assist with organization of quarterly aftercare event recognizing clients who have obtained/maintained permanent housingCollaborate closely with program directors and social service team members to ensure program needs are being met and provide resources as neededAssist clients with post-aftercare services transitionAt least 40% of this position is in the field meeting with landlords/brokers and conducting in-home aftercare visits with former shelter residentsAdditional duties as assigned by supervisorEssential Physical Job Functions:Capable of walking up and down multiple flights of stairs throughout the day Able to stand for long periods of time as neededAbility to work onsite in the Lower East Side and travel throughout the 5 boroughs of NYCusing public transportation.
