Job DetailsJob Location Main Campus - Syracuse, NY Position Type Full-Time/Part-Time Education Level Master's Degree Salary Range $66,000.00 - $70,000.00 Salary/year DescriptionPosition Statement: The Aftercare Clinician is responsible for providing clinical services to youth and families who are being discharged from an Elmcrest residential program. This includes a variety of activities, ranging from providing individual, group, and family therapy, to communicating the clinical needs of youth to various stakeholders (e.g., collaborative and or partnering agencies, families, counties, courts, etc.). The Aftercare Clinician functions as the primary individual responsible for continued treatment of a youth transitioning into the community setting. The Aftercare Clinician's role is to assist youth along with their family with the adjustment of being back in the community setting. General Responsibilities:
- Serve as the clinical lead in providing and coordinating the therapeutic services needed by youth and families by traveling to and from their home and/or community setting.
- Consistently interact with youth, families, and staff in a professional manner that reflects favorably on the organization and that is conducive to effective outcomes.
- Maintain a positive rapport and professional relationship with families and county representatives (such as caseworkers and law guardians).
- Understand and consistently implement all Agency approved policies, protocols, and procedures.
- With the immediate supervisor, set and achieve annual goals for job performance improvement.
- Maintain timely and accurate record keeping and reports as required for assisting youth and demonstrating adherence to job expectations. Demonstrate accurate use of grammar, spelling, and syntax for correspondence.
- Meet established paperwork timelines.
- Meet established attendance criteria for meetings, events, etc.
- Perform duties assigned by the Aftercare Coordinator, including, but not limited to, those listed below:
- Provide individual, group, and family therapy within the community.
- Maintain consistent contact with family members, other service providers, and county representatives.
- Complete clinical paperwork and assessments (e.g., treatment plans, lethality and risk assessments).
- Meet minimum required billables per week.
Qualifications
- Master's Degree or Licensed Professional in mental health field (LMSW, LMFT, LMHC, etc.)
- Experience and familiarity working with/in mental health environment.
- Knowledge of community resources and support systems.
- Ability to work independently and to collaborate effectively with a multidisciplinary team.
- Valid Driver's License, proof of automobile insurance.
