Jacksonville, FL 32205 Under the supervision of the Director of Nursing or designee, the Home Health Aide performs basic patient care activities necessary in caring for the personal needs and comfort of patients. Weekend and holiday work is required. Duties and Responsibilities:
- Performs personal care activities to promote and maintain patient hygiene.
- Assists patients in performing necessary activities of daily living.
- Assists patients with ambulation and exercise.
- Uses proper body mechanics to promote patient safety.
- Maintains a safe patient environment to prevent patient injury.
- Provides care in an emotionally supportive approach.
- Provides nutritional support to patients.
- Participates in mentoring new employees.
- Works with all departments to ensure compliance with objectives, plans and procedures.
- Maintains a neat and clean environment while carrying out all duties.
- Provides light housekeeping, and meal preparation as directed in the patient plan of care.
- Does not deviate from patient centered plan of care.
- Demonstrates knowledge of standard precautions and ability to perform basic infection control procedures.
- Demonstrates to family/caregiver proper techniques to meet the personal care needs of the patient.
- Reports changes in patient's condition or care needs that require follow-up to supervisory and/or appropriate team member.
- Documents care provided in accordance with organization standards.
- Maintains a minimum of twelve (12) hours of training/education specific to position annually.
- Always maintains patient confidentiality.
- Additional duties, as assigned.
Qualifications and Requirements: - High school diploma or GED equivalent preferred.
- Current licensure as a Certified Nursing Assistant (CNA) in the state of Florida required.
- Current State of Florida Driver's license with driving record acceptable to company's insurance carrier; reliable transportation.
- Current Auto Insurance Card
- Current CPR Certification from an instructor or training provider approved to provide CPR by the
- American Red Cross, the American Heart Association, the National Safety Council, or an organization whose training is accredited by the Commission on Accreditation for Pre-Hospital Continuing Education.
Skilled at: - Demonstrates personal care skills.
- Ability to communicate and establish rapport with patients/families.
Medical Clearance - Current TB testing and immunizations/declination for Hepatitis B and Flu
