Job Description:
Jacksonville, FL 32205The Social Worker (MSW) provides social work services to home health patients and their families. Counsels and provides support for patients' psychosocial needs during illness and treatment. Advises family members about additional services for Alivia Home Health (AHH NE) and Alivia Care, Inc. and makes referrals. Participates as a member of a multi-disciplinary team and assists with the development of a holistic treatment plan.Duties and Responsibilities:
  • Demonstrates support for the agency mission statement and philosophy to promote quality, efficient, comprehensive, compassionate, and effective patient care
  • Responsible for assessing the psychosocial status, establishing, monitoring, and delivering care as it relates to meeting the psychosocial needs of the patient as directed by the physician's plan of care.
  • Assist the physician, physician assistant, or advanced practice registered nurse and other members of the health team in understanding significant social and emotional factors related to the patient's health problems.
  • Responsible for the provision of social work services including short-term individual counseling, community resource planning, and crises intervention.
  • Collaborate with the interdisciplinary team to develop a comprehensive care plan
  • Maintain regular attendance at and participate in Interdisciplinary Team meetings, communicate patient changes, and collaborate with team members in care planning decisions and coordination
  • Provide ongoing support, counseling, and education to patients and family regarding a variety of issues. Help the patient and caregiver to understand, accept and follow medical recommendations and provide services planned to restore the patient to optimum social and health adjustment; Assist patients and caregivers with personal and environmental difficulties which predispose toward illness or interfere with obtaining maximum benefits from medical care.
  • Act as a facilitator for meetings with patient, family, caregivers, and community agencies to clarify or problem solves issues, including a plan of care. Mediate discussions between all parties.
  • Attend and actively participate in a variety of organizational meetings related to patient care, daily operations, in-services, and community agency meetings.
  • Act as a resource to other team members regarding the social, emotional, and ethical factors related to health problems.
  • Complete all documentation according to Alivia Home Health (AHH NE) documentation requirements.
  • Act only within the scope of his or her authority to practice. May not provide clinical counseling unless licensed to do so.
  • Follow all Policies and Procedures and OSHA safety guidelines.
  • Protect privacy and maintain the confidentiality of all company procedures, results, and information about employees, patients, and families.
  • Practices standard precautions.
  • Maintain a safe working environment, following Alivia Home Health (AHH NE) safety policies and procedures.
  • Participate in and support Quality Improvement initiatives
  • Participate in continuing education classes and any required staff and training meetings.
  • Maintain professional affiliations, required certifications, and continuing education requirements.
Qualifications and Requirements:
  • Master's degree in Social Work from a school of social work accredited by the Council on Social Work Education, required
  • One year of social work experience in a healthcare setting required
  • Current driver's license and dependable insured automobile
  • Current CPR certification
  • Level II ACHA background screening
  • Working knowledge of home health care regulations and procedures
  • Excellent communication skills and highly engaged team member
  • Works will in a team comprised of a variety of backgrounds and educational levels
  • Possess good time management, problem-solving, and high-level work ethic

  • Ability to communicate and articulate Alivia Home Health (AHH NE) services and benefits to medical professionals in a clear and concise manner
  • Ability to work autonomously yet support the needs of Alivia Home Health (AHH NE) and your fellow team members
  • Computer proficiency with Microsoft Office including Word, Excel, Outlook, and PowerPoint
Medical Clearance
  • Current TB testing and immunizations/declination for Hepatitis B and Flu
