ESSENTIAL JOB RESPONSIBILITIES:

- Maintains knowledge of Alzheimer's and dementia and current issues related to Hospice and Supportive Care.
- Provides Virtual Dementia Tour and Teepa Snow Positive Approach to Care training for HMC new hires.
- Provides Virtual Dementia Tour and Teepa Snow Positive Approach to Care training for medical students/residents who shadow the Chief Medical Officer.
- Facilitates the Dementia Caregiver Workshops.
- Provides effective communication to the medical community, community at large, facility staff and/or caregivers regarding Alzheimer's/dementia and HMC services and programs.
- Provides Alzheimer's and dementia education/in-service presentations to the medical community, community at large and facilities.
- Successfully educates/markets HMC services and provides informational visits that lead to early referrals of appropriate Alzheimer's and dementia Hospice patients.
- Serves as a clinical liaison for the High Utilizer Task Force.
- Sits on the Baker Act Task Force.
- Schedule and collaborates educational opportunities with 55+ community residents (OTOW, Del Webb, SummerGlen, Stone Crest, Spruce Creek, Ocala Preserve, Ocala Palms, Oak Run, Pine Run, etc.) for Alzheimer's and dementia education.
- Acts as a representative of the Nancy Renyhart Center for Dementia Education at community healthcare events.
- Collaborates with the Education, Marketing and Volunteer Departments, as needed.
QUALIFICATION REQUIREMENTS:

- Minimum of A.A. in nursing, with BSN, preferred. Florida RN license in good standing. 2-4 years of medical/surgical, oncology experience and prior Hospice experience, preferred.
- Must have current CPR.
- Must have a valid Florida driver's license, maintain current auto insurance coverage and have access to a reliable automobile.
- Ability to work unsupervised in the field, exercising discretion and independent judgment.
- Ability to read, analyze and interpret general business periodicals, professional journals, technical procedures and governmental regulations, as well as document such as safety rules, operating and maintenance instructions and procedure manuals.
- Ability to write routine reports and correspondence.
