We are also highly-ranked by Castle Connolly Medical, a healthcare rating company for consumers, among the top 5 leading U.S. medical schools for training doctors.Bargaining Unit:UUPJob Summary:The Department of Pharmacy at SUNY Downstate Health Sciences University is seeking a full-time Ambulatory Care Clinical Pharmacist.The Ambulatory Care Clinical Pharmacist will provide pharmacy services for patients in adult outpatient clinics at the State University of New York (SUNY) Downstate Health Sciences University.The specialist will work as a part of various multidisciplinary teams in order to deliver safe, appropriate, comprehensive and cost-effective drug therapy for patients post-discharge or with chronic medical conditions. Medication reconciliation and medication education will be paramount in ensuring optimal pharmacotherapy. Therapeutic areas of focus include anticoagulant and antithrombosis, tobacco cessation, COPD, diabetes mellitus, heart failure, and general medicine/pharmacotherapy.The Ambulatory Care Clinical Pharmacist will also serve as a Pharmacy Department liaison and drug information resource to providers, nurses, and pharmacists.Other duties and responsibilities include:
- Reviewing medication profiles of patients seen in outpatient clinics
- Performing medication reconciliation at clinic visits to determine appropriateness of medication orders and to assess patient adherence
- Collaborating with physicians to optimize drug regimens
- Documenting interactions with patients in the electronic health record
- Identifying barriers to medication adherence and implement measures to facilitate access to medications whenever necessary
- Providing education programs for physicians, nurses, pharmacists, patients, pharmacy students, and pharmacy residents
- Precepting pharmacy students and PGY 1 pharmacy residents
- Representing the Pharmacy Department in committee meetings and hospital medication safety initiatives
- Assisting in pharmacy and hospital initiatives to provide safe and efficacious evidence-based care
- Assisting in the development of policies and procedures to support safe medication use, storage, and security in ambulatory care areas for regulatory compliance
Required Qualifications:
- Doctorate of Pharmacy (PharmD) from an accredited College of Pharmacy
- New York State Pharmacist Licensure or eligible for NYS Pharmacist license
- Completion of PGY 1 and Pharmacy Residency
- Completion of PGY 2 and Pharmacy Residency in Transition of Care or Ambulatory Care
Preferred Qualifications:
- New York State Collaborative Drug Therapy Management Certification
- Medication Therapy Management Certification
