CareStar, Inc. is currently seeking an Assessment Specialist Flex in the Clinical department to conduct program assessments using program and CareStar identified tools and processes, and provide recommendations for eligibility per program and CareStar guidelines.THE POSITIONCareStar is looking for motivated individuals who: Hold an active license as a Licensed Social Worker, Licensed Independent Social Worker, or Registered Nurse.Have experience working with the populations served by the applicable program and experience in the home and community-based services environment.Possess effective oral and written communication skills with attention to detail in assessment and documentation; ability to prioritize multiple tasks; and proficient computer skills with a suite of Microsoft Office programs.Can work independently within CareStar and program guidelines, demonstrating effective interpersonal skills within a diverse work environment, strong problem-solving skills, effective organizational skills, and flexibility in relation to position requirements.Adhere to the CareStar Rule in performance of job responsibilities.Understand and comply with CareStar Policies and Procedures.Maintain confidentiality as related to patient information. Any unlawful disclosures of confidential information made outside the proper course of duty will be treated as a serious disciplinary offense.Follow the Acceptable Use Policy while using any information systems owned or controlled by CareStar, Inc.
