This position directly supports the Nursing Home Administrator in overseeing all aspects of facility operations. Additionally, the role is responsible for developing, implementing, and monitoring quality assurance and continuous quality improvement initiatives. The position ensures that Lakeland Health Care Center is always prepared for recertification and complaint surveys. Collaborating with facility leadership, this role works to systematically identify and address any deficiencies.

SUPERVISION

This position supervises facility leaders when the Nursing Home Administrator is unavailable.

SCHEDULE

Days: Monday - Friday

Work Hours: 8:00 am - 4:30 pm*

Hours Per Shift: 8

Shifts Per Pay Period: 10

Position FTE: 1.0

* Hours of Work: This position is typically scheduled from Monday to Friday, with weekends off. However, an individual in this role may occasionally be required to work outside of normal hours to accommodate supervisory needs.

Assist Nursing Home Administrator with the management of day-to-day facility operations.

Assume the responsibilities of the Administrator in the absence and at other times delegated.

Provide guidance and leadership to department managers in the development quality assurance and quality improvement efforts in areas such as resident care, workforce development, budget development and management.

Partner with all departments to create, review, and update necessary policies and procedures. Assure that all policies meet state and federal requirements.

Work with the Administrator, Director of Nursing and Human Resources department to develop a yearly Employee Recruitment and Retention Plan.

Lead the abuse investigation process. Prepare and submit self-reports to the Wisconsin Department of Health Services.

Ensure that the facility remains in a survey-ready status. Plan and develop a survey plan to identify areas of vulnerability. Develop plans to address vulnerabilities.

Assist with state survey process and preparedness. Assist in the formulation of plans of correction when/if citations are imposed.

Function as the Compliance Liaison and ensure that the Corporate Compliance Agreement requirements are met by the facility.

Lead the Safety Committee and conduct monthly safety rounds to ensure all CMS life safety regulations.

Serve as liaison to the Public Works Department. Work with maintenance staff department and members of the safety committee to conduct regularly scheduled emergency drills.

Lead the Quality Assurance Performance Improvement meeting and create Performance Improvement Plans based on the areas that need further development.

Demonstrate a commitment to county safety and risk management efforts by working to help reduce the risk of injuries within the facility.

Other duties and responsibilities given by the Director of Nursing or Administrator.

Education and/or Experience

Bachelor's degree in a health-related field with nursing home experience preferred and least two to three years of experience in a long-term care setting; or an equivalent combination of education and experience. Candidates must possess the ability to coach, mentor and consult with a wide range of employees as well as the ability to work with mathematical concepts such as probability and statistical inference. Strong computer proficiency, excellent interpersonal, communication and influencing skills are essential for success.

Provides specialized information and/or recommendations to others regarding an area of expertise. Required to actively persuade and/or direct others toward the achievement of expected outcomes. Strong written, verbal and interpersonal skills required.
