Job DescriptionSUMMARY OF DUTIES AND RESPONSIBILITIES:The Assistant Director for Community Engagement, Street Health Outreach & Wellness Program in the Office of Ambulatory Care will be responsible for building and maintaining relationships with community organizations and partner city agencies that work in the spheres of harm reduction, street homelessness, and medical care.General tasks and responsibilities will include:•Liaise with key community partners working with SHOW target population to identify mobile clinic locations; help identify populations in need of SHOW services; and collaborate on new or improved program offerings•Identify, initiate, manage, and foster trusted relationships with community organizations and local stakeholders•Create focused community partnerships in line with SHOW goals and the social, cultural, and linguistic needs of the patient population•Foster collaborations between SHOW and community partners on particular patient needs to streamline connection to services•Facilitate quality improvement activities with SHOW staff and community partners as needed•Continuously expand knowledge and understanding of community resources, services, and programs by attending community meetings, events, and community advisory committee meetings to understand community issues or build relationships with community members.•Collaborate with the Data Analyst and other members of the Central Office program team to develop reports and presentations with the goal of communicating program progress to internal/external stakeholders•Represent the Program at Facility meetings, events, and functions as needed•Participate in staff meetings and trainings as required•Perform other duties as assigned.
Minimum QualificationsEducational and/or experience requirements will vary requiring diverse kinds and levels depending on the content, complexity and variety of project.Compensation levels will correspond to the requirements developed for the designated project. In no event will the acceptable minimum be below: 1. A Baccalaureate Degree from an accredited college or university in a specialization discipline consistent with project content; and, 2. Four years of substitutable and relevant health care experience related to the objectives of the project; or, 3. A satisfactory equivalent combination of experience, training and education.
