Pay Range: $70,000-$85,000 Exempt (Pay information will vary depending upon relevant experience for the position)Full Time Monday-Friday 8:30am-5pm with rotating on-callGENERAL SCOPE: Experienced LPN or RN leader to supervise and mentor care staff to provide quality care to assisted living residents in a family-like environment. Team player, positive attitude, organized. Ensures care is delivered per current federal and state regulations and ensures that the resident's individual health needs are maintained. EDUCATION / QUALIFICATION: Current NYS Licensed Practical Nurse (LPN) or current NYS Professional Registered Nurse (RN) License required. Bachelor degree in healthcare services or related field preferred.Knowledgeable of nursing and medical practices and procedures, as well as laws, regulations and guidelines that pertain to assisted living. Possess leadership and supervisory skills and the ability to work collaboratively with personnel. Plan and implement processes for providing high quality assisted living, enhanced assisted living, and special needs assisted living care. Three years clinical experience preferred.ESSENTIAL FUNCTIONS:
- Oversees direct care staff including LPNs, Resident Assistants, Program Coordinator and Medical Technicians.
- Performs nursing assessments as required for the care of residents and oversees wellness plans for residents.
- Assesses and evaluates changes in the health status of residents and communicates changes in resident status to appropriate physicians, staff, leadership, families or other appropriate parties.
- Develops and implements an interim plan of care in consultation with the physician when appropriate.
- Collaborate with Case Manager for appropriate level of care, participates in care conferences with families as needed.
- Assesses performance of assigned staff and encourages opportunities for education and advancement of staff.
- Serves as the primary clinical nursing resource for nursing staff on assigned shift.
- All other duties as assigned.
EXPERIENCE: One (1) year of supervisor or management experience and three (3) to five (5) years experience as a Registered Nurse with three (3) years clinical experience preferred.
