Our focus is supporting individuals by ways of residential housing, mental health and clinical services, and immersing our participants in community engagement. Working closely with the Office for People with Developmental Disabilities and the Office of Mental Health, we support 27 housing locations and 3 administrative buildings. Main Function: Under the supervision of the Program Manager, use experience and professional judgment to successfully carry out the mission of the Children's Intensive Respite Center Provide person centered services and supports to children with complex needs to achieve increased independence and maintain their health and safety. May be providing support to children with significant mental health and/or behavioral needs. Responsibilities include: promoting growth and independence with personal needs and ADLs, implementing behavioral support plans, monitoring health and safety, building advocacy, decision making and community connections, and completion of required documentation.Duties and Responsibilities:1. Assist Program Manager with the day to day operations of the Intensive Respite program
  • Assist Program Manager with the oversight of the implementation of child's staff action plan, IPOPs and behavior plans by Children's Respite Specialists.
  • Assist Program Manager with oversight of medical monitoring of each child, including attending medical appointments as directed
  • Provide oversight and guidance to Respite Specialists in the absence of the Program Manager
  • Assist the Program Manager with staff training
  • Shares on-call responsibilities with Program Manager
2. Provision of services for child/family
  • Create a supportive and therapeutic environment that promotes emotional regulation and stability, fostering a sense of security and belonging for children in crisis
  • Implement child's goals as written in Staff Action/Service Plan
  • Assist/support child with ADL skills including, but not limited to, bathing, toileting, dressing, dining, mobility, as needed
  • Teach child skills to promote independence including, but not limited to: reduction of target behaviors, socialization and health and safety
  • Implement behavior guidelines/plans as written
  • Engage children in a variety of person-centered recreational activities designed to promote positive coping skills, socialization, and personal growth
  • Offer child choices and encourage them to make healthy choices
  • Transport and accompany child to activities of their choice
  • Monitor and maintain participants' safety
    • Work collaboratively with interdisciplinary team, including clinicians, educators, healthcare professionals, and family to develop and implement a person-centered crisis intervention plan tailored to each child's specific needs, goals and preferences
    • Monitor and maintain the safety of the child while in the home, community, and during transportation
    • Report incidents involving the child per Agency standard
    • Utilize pro-active approach to avert and/or de-escalate crisis situations
    • Follow procedures to report emergencies or medical/safety needs/changes of the child
  • Completes required documentation
    • Complete all program documentation in a timely manner and per Agency standards. Documentation includes but not limited to: service notes, behavioral data, program outcomes, etc.
    • Completes incident report forms thoroughly and in a timely manner per Agency standards
    • Completes mileage sheets per Agency standards
  • Community and Teamwork
    • Work collaboratively with the child, family, program staff, clinicians, hospital staff, co-workers and supervisor
    • Work collaboratively with other providers, schools, etc. to meet the need of the child/family
    • Act as an advocate for child/family as needed or requested
  • Follows Agency standards and procedures
    • Provides person-centered, quality services
    • Obtains and demonstrates core competencies
    • Follows Agency standards and procedures
    • Maintains compliance with applicable rules and regulations
    7. Other duties as assignedEducation/Experience:High school diploma/GED required. One year experience working with children with I/DD or a Mental Health diagnosis preferred. Registered Behavior Technician (RBT) certification or willingness to obtain preferred. Experience working with children with behavioral needs preferred.Conditions of Employment: Must have a valid New York State driver license in accordance with the Agency Vehicle and Driving Policy and transportation necessary to meet program needs. Must be able to demonstrate teamwork and professionalism. Must be willing to visit children and families in their home setting, travel outside Onondaga County may be required. Must be able and willing to demonstrate direct care duties, including personal assistance. Must be able to lift 50 pounds.
