**Job Description:**
The position coordinates the delivery of services in the Home Care setting. In collaboration with the manager, the Coordinator is responsible for ensuring that Home Care services are provided to the ultimate satisfaction of patients, meeting the expectations of referral sources and other external customers. The coordinator also ensures that the continuum of care is maintained as each patient moves through the system.

**Minimum or Preferred Qualifications:**
AAS - Associate of Applied Science of Nursing OR BS - Bachelor of Science in Physical or Occupational Therapy required. BS - Bachelor of Science in Nursing OR MS - Masters of Science in Physical or Occupational Therapy preferred. 2 years of experience in home care required. Licensure corresponding with degree (Registered Nurse, Physical Therapist, Occupational Therapist) required. NYS drivers license required upon hire.

**Salary Range:**
