Under the direction of the site supervisor, assists the overall operation of an Arc residence including providing on-site supervision of direct care staff to include week nights, holidays and week-end hours based on program needs.
Job Description
Assist in the implementation of the Individualized Service Plan (ISP) as developed for each consumer by his/her family advocate and Service Coordinator
Ensure that the consumer's medical needs are met by scheduling medical appointments, making arrangements to ensure that the consumer gets to the appointments, communicating all necessary information to nursing staff, the Service Coordinator, family advocate, residential staff and scheduling any follow-up or referral appointment as needed
Assist in the completion of the monthly paperwork accurately (i.e. attendance records, petty cash, med fund, gas fund, monthly receipts, personal allowance records, etc.)
Ensure residential habilitation services are documented in accordance with the Agency and OPWDD requirements
Ensure residential habilitation services are maintained and required transportation paperwork is complete, per policy
Working under the direction of the site supervisor for hiring, orientation and ongoing training, supervision and evaluation of all direct care staff
Under the direction of the site supervisor, schedules direct care staff ensuring that the number and types of staff assigned are adequate to meet the needs of the consumers
With the site supervisor responsible for ensuring that there is a supervisor on staff 7 days a week during key program hours (weeknights, holidays and weekends)
Become certified in medication administration and maintain certification on an annual basis
Provide direct care services, as needed
Provide on call services including holidays, weekends and evening, as needed when site supervisor is not available or on call
Assist in ensuring that adequate and appropriate recreational activities in the community and scheduled for consumers year round
Coordinate transportation as needed for consumers
Ensures that the nutritional needs of the consumers are met
Participates in corporate compliance and quality assurance activities as required
Other related duties as assigned
Knowledge, Skills, & Abilities:
Requires attentive listening skills, excellent oral and written communication skills and good reading abilities.
Basic computer skills
Ability to maintain composure during emergency/conflict situations and be able to use sound judgment in decision making.
Ability to relate to people of different abilities, backgrounds and cultures
Ability to work independently.
Physical & Mental Requirements:
Be physically able to assist individuals in transferring from wheelchair to other seat when needed
May be required to prevent an adult from falling, assisting an adult to his/her feet, etc. (adults weighing up to 200 pounds, plus)
Walk up and down stairs.
Shovel and remove snow and ice from walkways and driveways when necessary.
Minimum Qualifications:
High School Diploma or equivalent
At one years experience with individuals with developmental disabilities
Certified Nursing Assistant or Certified Home Health Aide preferred
Valid NYS drivers license with a safe driving record per agency policy
Prior supervisory experience preferred
Working Conditions:
Will be required to use office equipment, i.e., phone, copier, fax in addition to household appliances, i.e., washer/dryer, stove/oven, refrigerator, etc.
Work may require offsite travel to appointment / events / outings.
Monday - Thursday 2:30pm-10pm
Sunday 7-2:30pm
