Job purpose:

Assist the Residential Facility Manager in the operation of the residence and perform duties to include, but not limited to scheduling staff, orientation of employees and participating in the Inservice program.

Duties and Responsibilities:

* Develop and implement habilitative programs for each individual's Plan of Service.

* Assist the Residential Facility Manager with case management responsibilities. May act in role of case manager.

* Maintain standards consistent with DDS licensing and/or all other certification requirements of a community residence.

* Supervise the Residential staff and program in the absence of the Residential Facility Manager.

* Receive direct supervision from the Residential Facility Manager.

* Develop programs and maintain timely, accurate, and consistent records to foster individual growth and development of each individual in the following areas: safety skills; self help skills; social skills; food management; household management; community living skills; money management; and leisure education.

* Serve as on call Assistant Residential Manager for the Kennedy Center community residences as required.

* Provide ongoing direct care to individuals served.

* Administer evaluations and assessments to individuals served.

* Administer individuals' medication/treatment in accordance with doctor orders and agency policies and procedures.

* Attend the required in services as well as other in services directly job related as specified in the agency's Policy and Procedures Manual.

* Update the team on a regular basis as to current programming issues and needs and revise schedule of staff programming responsibilities within the home.

* Knowledgeable of and insure the protection of each individual's rights in accordance with agency policies and procedures.

* Maintain contact with parents/guardian, employer, job coach and other personnel as assigned.

* Transport individuals for medical examinations, recreational and financial appointments as requested.

* Maintain communication and contact between programs, IDT members, parents and/or guardians as needed.

* Audit individuals' main case file quarterly and maintain records.

* Other related duties and responsibilities as assigned.

Qualifications:

* High School Diploma.

* One year of experience working with persons with disabilities.

* Valid Driver's License and ability to possess a State of CT Public Service License.

* Must maintain a good driving record.

* You are required to be in possession of a car on site during your entire shift to transport yourself and/or individuals served to community sites in all weather conditions and throughout all neighborhoods.

* Depending upon assignment, must be able to work in a variety of settings and locations.

* Must maintain PMT, CPR and DDS medication certification.

* Knowledge and skills in Microsoft Office and the internet.

Physical Requirements:

* Work a flexible schedule that may include early mornings, evenings, weekends and/or holidays.

* Provide hands on personal care which may include toileting, feeding, cleaning and all other activities of daily living skills.

* Ability to transfer individuals requiring personal hygiene care.

* Have physical endurance to use physical crisis intervention techniques which may include restraining individuals.

* Provide physical interventions which may include lifting, mobility, first aid, and safety evacuations.

* Communicate clearly in oral and written form on a constant basis.

* Climb stairs and move weights up to 20 lbs. on a consistent basis and up to 40 lbs. on an occasional basis.

* Work independently; utilize good judgment and possess a strong sense of initiative.
