Summary of Position

The licensed clinician works with primary care providers to ensure that the medical and psychosocial needs of patients are addressed. Our goal is to improve patient health outcomes through quality care and integrated caseload-focused consultation. This licensed clinician will provide licensed clinical work through an integrated, team-oriented, healthcare approach to children, adults, and families or groups regarding psychological or emotional problems. This licensed clinician will develop plans with the patient and implement therapeutic treatment plan in the health centers. The licensed clinician will provide culturally sensitive mental health services by providing assessments, referrals, and follow-up appointments. Responsible for informing, educating, advocating for, and referring patient to local social agencies and services. This position may also assist with clinical supervision of non-licensed behavioral health staff and trainees and provide behavioral health education to PTH staff.

Qualifications & Work Experience

Licensed and in good standing with the California Board of Behavioral Sciences as an LCSW or LMFT. Experience providing behavioral health services (i.e., assessment, diagnosis, individual and group therapies) to homeless, mentally ill, substance addicted adult populations and/or children, youth, and families from low-income neighborhoods. Prior experience working within a community mental health or primary care, medical setting preferred. CPR certification required. Proficiency in computer use required. Proficiency in written and spoken English, as well as preference in Spanish. A minimum of three years' experience, preferably working in a community clinic setting or a mental health agency that serves a wide variety of patients. Supervisory experience a plus.

Education

Master's Degree in Social Work, Psychology or related field. Knowledge of integrated behavioral health; mental health recovery, harm reduction, age specific growth and development, crisis and behavior management is expected. Working knowledge of administrative practices and procedures, HIPAA regulations, policies and standards related behavioral health.

Examples of Duties

*

* Provide short-term, structured, evidence-based interventions as part of an appropriate treatment plan, which may include individual and/or group therapy, and possible coordination of treatment for participants with co-occurring disorders.

* Assesses client needs, abilities or behavior using a variety of methods, including interviews and direct observation of behavior to assess and review their situations.

* Flexibility in meeting with patients (in person, on a virtual platform, or telephonic).

* Uses appropriate techniques for the evaluation and treatment of patients in medical clinic environment.

* Provide evidenced based practices to individuals, couples, families, and groups.

* Collaborate and coordinate with other members of health care team, in particular the client's PCP, to provide holistic, well-integrated health care in order to improve the outcome of patients.

* Provide consultation services to multidisciplinary team members by evaluating the biopsychosocial implications of the patient's illness when the team is assessing and planning medical treatment.

* Documents and reviews documentation of integrated health visits, such as, demographic data, treatment plans, psychosocial history, progress notes, discharge planning, etc.

* Oversees and assists each assigned client with individualized service planning and reviews.

* Develops and implements intervention and treatment programs that the patients can interpret and comply with.

* Provides culturally sensitive therapy modalities and develops and provides new services.

* Assists in clinical discharge planning, referring clients to, or providing alternative services.

* Facilitate referrals to other services, such as substance-abuse treatment, specialty care, and community resources, as needed.

* Provide crisis intervention as needed.

* Advocate for clients and work to resolve individual client barriers in order to facilitate and encourage follow-up and collaborative, interdisciplinary care.

* Participates in staff meetings and other quality improvement meetings as needed.

* Maintain accurate and timely electronic health records of activities, and services provided to each client.

* Submit billing daily that is accurate and complete.

Coordination of Intern Program

* Attend college fairs for recruitment of intern

* May assist with programming and educational curriculum for interns

* May serve as a supervisor to interns

* May provide evaluations and chart reviews for interns

* Provide shadowing opportunities to interns

* Perform other duties as required

Additional Responsibilities

* May provide training and professional development of staff.

Travel Requirements

May regularly travel to other sites to provide patient care and/or attend meetings or training; occasionally overnight for training. Having flexibility to work in multiple locations is expected.
